What is trigger finger?
Trigger finger is a condition in which the finger is temporarily or permanently blocked with flexion and extension of the fingers, making normal functioning impossible.
Most people experience trigger finger in the fourth finger (ring finger) or in the thumb, the most frequent locations. Although the pathology can affect any of the fingers.
It is the result of inflammation (most frequently due to overuse, and therefore due to a chronic condition) in the flexor tendons or in the tissue around them.
More specifically, it occurs due to the involvement of the “pulleys”, which are bands of tissue that attach the finger bone to the flexor tendon.
Inflammation of the pulleys affects the ability of the finger to move freely.
What are the causes of trigger finger?
One or more of the following factors may be responsible:
- Certain medical conditions: diabetes and rheumatoid arthritis are known causes of trigger finger.
It is estimated that 10% of people with diabetes have trigger finger, compared to 2-3% of the general population.
- Excessive use of the fingers: people who work with their hands, especially those who perform strenuous activities with machinery or tools that require gripping, have a higher risk of developing the pathology.
- Contact friction: sometimes, repeated use of power tools that vibrate in the hand, or even gripping bicycle handlebars can cause trigger finger.
- Sometimes, a person develops trigger finger for an unknown reason (idiopathic).
This pathology or condition most often affects people between the ages of 40 and 50. Women are six times more likely than men to suffer from trigger finger according to studies.
Trigger finger symptoms
Symptoms of trigger finger are usually as follows:
- Visible or tender lump at the base of the finger at different levels of the phalanges.
- A finger that locks or makes noises with movement (flexion and extension).
- Pain and discomfort when straightening or bending the finger.
The person usually notices that their symptoms worsen after a period of heavier hand use.
In addition, symptoms are usually more intense in the morning, because the flexor musculature (as well as the rest) is not active during the hours of sleep.
This is a vicious circle: the spring finger hinders mobility, while the lack of mobility increases the stiffness and thus the severity of the spring finger.
Early diagnosis and treatment can have very satisfactory results.
Diagnosis of trigger finger

Treatment of trigger finger
Conservative treatment of trigger finger
Initially we will start with RICE treatments, based on rest, ice application, compression by bandaging and elevation.
- Apply ice to the affected toe area to reduce the initial swelling.
- To eliminate pain and inflammation, it is possible to modify or limit activities, avoiding any type of movement that involves repetitive use of the finger.
- We may also recommend non-steroidal anti-inflammatory drugs (NSAIDs), such as aspirin and ibuprofen, to relieve pain and prevent inflammation.
Physical therapy exercises
Exercises to stretch and strengthen the phalanges, as well as neighboring joints, can help people with spring finger and those with a history of the condition prevent and reduce pain.
Although the exercises may seem simple and even have some doubt as to their outcome, they are very effective in helping the patient relieve stress and tension in the fingers and hands.
By means of a rehabilitation and physiotherapy protocol, it is possible to obtain a correct result, or on the other hand, to improve the functionality of the phalanges after the application of other treatments as a whole.
Corticosteroid injections
Dr. Alfonso Porcar may recommend the injection of corticosteroids around the tendon sheath of the affected finger.
These drugs can help reduce the incidence of trigger finger pain and limit movement impairment.
Sometimes, you may need two or three injections to experience symptom relief.
People with trigger finger have seen long-term relief after a second or third trigger finger injection.
Trigger finger release surgery
If the patient, despite conservative treatment, exercises and corticosteroid injection does not achieve an appreciable clinical improvement, we may recommend surgical intervention, especially in cases where the toe is permanently “trapped” out of its usual position.
Two treatment options are available:
- Open surgery by outpatient procedure: surgery usually lasts less than half an hour.
Local anesthetic plus sedation is administered. Then, on the affected hand, a minimal incision is created to release the pulley that affects finger movement.
- Percutaneous release: only applicable for triphalangeal fingers (2nd-4th).
After numbing the hand, a needle is inserted into the skin and around the injured tendon. This needle then breaks the blocked area. Ultrasound is used to identify the correct location.
There is no incision, and it is performed in the office, without the need to go to the hospital.
Is spring finger surgery painful?
The operation of spring finger or stenosing tenosynovitis of the flexor tendon is usually not very painful. The most uncomfortable thing for the patient is usually the anesthesia, which is completely tolerable.
After the operation, full mobility of the fingers is allowed and physiotherapy and rehabilitation treatment is started to obtain full extension of the palm and fingers of the hand.
