Intraosseous PRP injections in hip, knee and shoulder
When knee osteoarthritis is severe, Ahlbäck grades 3 and 4, the subchondral bone causes pain and limits function.
Small cracks or microfractures develop and are easily seen on MRI which illuminates as edema.
Bone edema and bone necrosis indicate that osteoarthritis is actively progressing, and patients are much more likely to require a total knee or hip replacement if we do not actively intervene.
In these cases, the intraosseous injection of PRP obtained by extracting the patient’s own blood is the treatment of choice.
In certain procedures, the use of intraosseous growth factors is performed simultaneously with rehabilitative therapy or minimally invasive arthroscopy, since both procedures can be performed together and this enhances their efficacy.
This also applies in the adjunctive treatment of osteochondral lesions to promote fibrocartilage regeneration.
Knee and hip osteoarthritis: intraosseous and intra-articular injections
The paradigms of understanding osteoarthritis have changed in recent years. They are currently focused on three aspects:
- Osteoarthritis is an inflammatory disease.
- The joint is an organ where articular cartilage, synovial membrane, synovial fluid and subchondral bone are connected.
- The subchondral bone is where the whole process of cartilage degeneration begins, so its surveillance is key in early stages in order to prevent and slow down osteoarthrosis.
It has been proven that, in patients with Tonis grade III and Ahlbäck grade III-Iv osteoarthritis, the combination of intraosseous infiltrations of Platelet Rich Plasma (PRP) has improved pain and functionality of the affected hip or knee, delaying the need for arthroplasty.

Do intraosseous injections hurt?
Most of the time, intraosseous (IO) injections are performed under sedation in an outpatient surgical centre, which helps to minimise pain.
What type of patients are candidates for intraosseous infiltration?
Candidates for intraosseous infiltration include patients with severe osteoarthritis, with or without bone marrow edema lesions, trabecular fractures, inflammatory bone pathology, and localized pain corresponding to their MRI findings.
Generally, these patients have not achieved the expected results with conservative treatments and wish to avoid joint replacement surgery or are not ideal candidates due to factors such as age, activity level, weight, or underlying medical conditions.
What do intraosseous infiltrations consist of?
The technique consists of a subchondral infiltration of Platelet-Rich Plasma (PRP) with growth factors into the acetabulum (5 cc) and the subchondral bone of the femoral head (5 cc) in the case of the hip.
For osteoarthritis of the knee, infiltration is performed on the subchondral surfaces of the tibial plateau and femoral condyle.
We then perform another ultrasound-guided infiltration of intra-articular PRP at 15 days and 30 days.
This technique has a powerful regenerative effect because it neutralises all inflammatory cytokines (IL-1β, TNF-α, etc.).
