What is hip tendonitis?
Hip tendonitis or gluteal tendinopathy, are a type of tendon disorder in the hip area and the gluteal region.
This disorder causes the tendon tissue to deteriorate and can lead to partial rupture, or even total rupture over time if not properly treated.
What is gluteal tendinopathy?
Gluteal tendinopathy is a common cause of hip pain, most commonly in women, who experience pain in the lateral hip area when standing or sleeping on their side.
Physical therapy exercises may help, although some people need other treatments to improve their hip tendinitis.
What parts of the body are affected by gluteal tendinopathy?
Gluteal tendinopathy affects the tendons that connect the gluteal musculature (around the hip joint).
These include the gluteus maximus, gluteus medius, gluteus medius and gluteus minimus, hamstring and quadriceps muscles, among others.
These muscles and tendons run from the hip bone (pelvis) to the greater trochanter. The greater trochanter is the ridge at the top of the thigh bone (femur).
Who suffers from gluteal tendinopathy?
Women over 40, especially those who have completed menopause, and sometimes associated with a lack of adequate blood circulation, are more prone to hip pain and gluteal tendinopathy.
The condition also affects younger people who run, ski and dance (among other sports or activities, where the hips are the protagonist of the movement).
Causes of gluteal tendinopathy
Gluteal tendinopathy can be caused by overuse of the musculature.
Potential causes include:
- Exerting excessive force on the tendons during sports physical activity.
- Inflammation of the tendon due to a fall or other type of accident.
- Weight gain or obesity.
- Inactivity or sedentary lifestyle.
What is the difference between gluteal tendinopathy, gluteal tendonitis and trochanteric bursitis?
There are several conditions that can affect the muscle groups and tendons of the hips and buttocks, causing pain, but their underlying causes and treatments are different.
- Gluteal tendonitis: it is produced by repetitive movements that cause small lesions in the gluteal tendons located around the hip, causing inflammation of the tendon at its insertion and pain when moving and rotating the hip.
You may have pain in the internal region of the hip that improves with sports rest, ice, and anti-inflammatory drugs in its initial phase.
- Gluteal tendinopathy: this hip pain is the result of a tendon injury that causes the tissue to rupture or deteriorate.
Rest usually does not help, but physical therapy exercises may relieve symptoms.
- Trochanteric bursitis is inflammation of one of the bursae that cushion the space between bones, muscles and tendons. Some people with trochanteric bursitis develop greater trochanter pain syndrome.
Treatments may include non-steroidal anti-inflammatory drugs, corticosteroid injections, treatment with PRP and/or Cytokine Rich Serum, and surgery as a last therapeutic resort.
Symptoms of hip tendonitis and gluteal tendinopathy
The most notable symptom of gluteal tendinopathy is moderate to severe hip pain.
This pain extends down the outside of the leg to the knee and/or calf.
You may also experience pain in the lumbar region of the back, groin and buttocks.
The pain usually begins in the greater trochanter, and may even be tender to the touch.
The pain gets worse when:
- You climb stairs or a slope.
- You get out of bed in the morning.
- You sit for a long time or cross-legged.
- You stand on one leg.
Prevention of hip tendonitis
These tips can reduce your risk of developing hip pain and gluteal tendinopathy:
- Be physically active.
- Modify activities according to the symptoms experienced.
- Avoid any repetitive sporting activity that puts a lot of pressure on the hips.
- Lose weight, if necessary.
- Practice yoga and/or stretching exercises to keep your hip muscles flexible.
- Treat low back pain and maintain good posture.
Diagnosis of hip tendonitis

Is there an injection for gluteal tendinopathy?
Yes, injections can be used to treat gluteal tendinopathy, with the most common types being:
- Corticosteroid Injections: These are administered around the gluteus medius tendon to reduce inflammation and alleviate pain. Studies have shown that ultrasound-guided corticosteroid injections can be effective, with many patients experiencing significant pain reduction within a month.
- Platelet-Rich Plasma (PRP) Injections: PRP therapy involves injecting a concentration of the patient’s own platelets to promote tendon healing.
