What is elbow pain?

The elbow is a joint formed by three bones: the humerus, the radius and the ulna.

The bones are held together by ligaments, muscles and tendon insertions to give stability and mobility to the joint.

Injuries to the elbow region are usually caused byoveruse of the elbow or acute injury.

  • Overuse injuries are common in people who perform repetitive arm movements for work, such as manual laborers and tradesmen, or for sports, such as tennis players and golfers.
  • Acute elbow injuries are less common, but can occur in some situations, such as contact sports, falling on an outstretched arm or some other accident.

Mild cases of elbow pain can be corrected with means of self-care and rest, but in some more severe cases of epicondylitis, epitrocleitis… treatments such as physiotherapy, regenerative medicine or surgery may be recommended.

Causes of elbow pain

Epicondyles

The epicondyle region refers to the area of the elbow where two bony prominences meet at the distal end of the humerus, called the lateral epicondyle and medial epicondyle.

  • Lateral epicondyle: Located on the outside of the elbow, it is the insertion point of the extensor muscle of the forearm. Inflammation of this area is commonly known as lateral epicondylitis (tennis elbow).
  • Medial epicondyle: Located on the inside of the elbow, it is the insertion point of the flexor muscles of the forearm. Inflammation of this area is known as medial epicondylitis.

Both epicondyles are important for elbow and forearm function, providing anchor points for tendons and muscles that allow movement and strength in the hand and arm.

Causes of elbow pain

Causes related to elbow injuries include damage to parts of the elbow, such as bones, cartilage, ligaments, tendons or muscles.

  • Arthritis in elbow: Occurs when the cartilage surface of the elbow is worn or damaged by previous injury, such as a dislocation or elbow fracture, or by degeneration.
  • Tennis elbow treatment: Lateral epicondylitis refers to intense pain in the external area of the elbow. It is the result of repetitive strain resulting in injury to the insertion of the tendons at the epicondyle of the elbow. Elbow infiltration with PRP is often the definitive solution for epicondylitis.
  • Golfer’s elbow: Also called medial epicondylitis, it causes pain in the inner aspect of the elbow (the medial epicondyle), which can radiate towards the inner aspect of the forearm. Its pathogenesis is similar to the previous one.
  • Elbow bursitis: Bursitis is the inflammation of the elbow bursa (a fluid-filled sac-like structure that reduces friction between bones, muscles and tendons). Direct traumas, a localized infection or an overuse, can be causes of the same one, among others.
Elbow pain symptoms

What are the symptoms of elbow pain?

Because the elbow joint is very complex, the symptoms of elbow pain can vary greatly.

Dr. Alfonso Porcar will review your history and examine you to diagnose the cause of your discomfort. Be sure to report any activities that make your elbow pain worse, as well as any other symptoms you have.

Symptoms that may appear are:

  • Dull pain at rest.
  • Pain when making a fist (golfer’s elbow).
  • Pain when opening the fingers (tennis elbow).
  • Reduced grip strength (press): difficulties and pain when trying to grasp objects, especially with the arm outstretched.
  • Localized and/or diffuse deformity and swelling (effusion).
  • Limited range of motion.
  • Heat and redness.

If you have suffered severe elbow trauma, seek immediate care. If mild to moderate elbow pain persists for more than seven to 14 days, make an appointment with an orthopedic specialist.

Risk Factors

The most common risk factors for elbow pain include any activity or occupation that involves frequent bending or straining of the elbow joint.

Participation in some high-contact activities, such as soccer, skiing and climbing, can also increase the risk of traumatic injuries, such as broken bones or tendons.

Some of the most common risk factors for elbow pain are:

  • The practice of sports such as baseball, soccer and other contact sports.
  • Practice of tennis, paddle tennis and other racquet sports, weight lifting, climbing and body weight exercises such as push-ups and pull-ups.
  • Golf practice .
  • Occupational occupations and participation in activities requiring repetitive and strenuous elbow action.
  • Trauma, such as falls or motor vehicle injuries

 

Diagnosis of elbow pain

Dr. Alfonso Porcar, expert in elbow pathologies and injuries, diagnoses elbow pain.

To do so, he will initially evaluate your medical history and then perform a physical evaluation.

  • A thorough clinical examination and, often, imaging are useful to accurately define the origin of the pain and, therefore, the best individualized treatment.
  • At Clínica Regenerat we use high resolution ultrasound. Ultrasound is useful to identify factors that can guide the best treatment (such as the severity of the pathology, the presence of inflammatory parameters and the effects of different ranges of motion).

It can also help to exclude other isolated or coexisting causes of pain, which would help us to differentiate the main pathology from other associated or distracting pathologies.

  • In cases where it may be necessary, we may order additional imaging tests, such as magnetic resonance imaging.

Infiltration treatment for elbow pain

Let’s now look at all the treatments available to treat epicondylitis and other common conditions in people experiencing elbow pain and injury.

Initially for most of the pathologies that cause pain in the elbow joint, we can start with a conservative treatment through the application of ice, rest, immobilization and elevation, in addition to the use of analgesics and non-steroidal anti-inflammatory drugs .

All this can be complemented with physiotherapy exercises.

Elbow infiltration with corticosteroids

Elbow infiltrations with corticosteroids can be a solution in lateral epicondylitis and lateral epicondylitis when there is a lot of pain, but should not be repeated if the problem is not resolved.

They can be effective, but they can increase the probability of relapse, so the case must be correctly selected.

Elbow infiltration with prolotherapy

Prolotherapy injections (dextrose solution) are obtaining very good results for the treatment of tendon and ligament injuries.

Multidisciplinary pain management for complex cases may include PRP injections and other forms of regenerative medicine prior to definitive elbow surgery.

 

Elbow infiltration: Platelet Rich Plasma PRP Treatment

  • PRP is a product derived from blood. PRP does not contain stem cells.
