What is shoulder pain?

Shoulder pain is a very frequent pathology in the population.

It can have innumerable causes and requires a very extensive study.

  • In young patients it is more frequently related to physical activity.
  • In middle-aged and elderly patients, it is more related to overuse and/or degeneration of the joint.

In addition to pain, shoulder injuries also cause stiffness, restriction of movement and difficulty in performing day-to-day activities.

causes of shoulder pain

Causes of shoulder pain

Causes related to shoulder pain include pathologies related to the humerus, clavicle and scapula bones, articular cartilage, ligaments, tendons and/or muscles of the region.

  • Shoulder tendonitis: Resulting from inflammation of one of the tendons that form the rotator cuff.
  • Rotator cuff tear: when inflammation is left untreated tendonitis can eventually lead to a torn cuff.
  • Arthritis in shoulder: occurs when the cartilage in the joint wears down and causes pain, stiffness and swelling.
  • Bursitis shoulder: is inflammation of the fluid-filled sacs or bursae that cushion the shoulder. It is usually consequence of tendinitis.
  • Frozen shoulder treatment: When the joint capsule thickens contracts and tightens more than it should. It is also called adhesive capsulitis.
  • Shoulder sprain: it is an excessive stretching of the ligaments, very common and usually occurs when falling or performing an overexertion.
  • Shoulder strain: is the stretching or tearing of a muscle and/or tendon (tissues that connect the muscle to the bone). It is frequent in people who practice sports sporadically.
  • Dislocated shoulder: Shoulder dislocation is a serious injury involving a momentary loss of contact between the bones of the shoulder joint.
  • Calcifications in the shoulder: appearance of small calcium deposits, usually in the supraspinatus tendon.
symptoms of shoulder pain

What are the symptoms of shoulder pain?

There are a multitude of manifestations of shoulder pain.

Some occur even when the shoulder is at rest, while others appear when we perform certain actions.

The severity and location of shoulder pain can vary, and depends primarily on the initial cause of the pain, whether it is an injury or a medical condition.

Signs and symptoms that may be associated with shoulder pain:

  • Heat or redness in the shoulder.
  • Neck, arm or back pain.
  • Clicking, popping, popping or grinding sensation when moving the arm.
  • Muscle stiffness and weakness.
  • Limited range of motion.

Risk factors for shoulder pain

Shoulder pain is usually caused by injury or overuse, both of which are more likely in those involved in sports and other high-risk activities.

Risk factors for shoulder pain are:

  • Constant or repetitive arm movements.
  • Participate in pitching sports such as baseball, football, softball, or basketball.
  • Advanced age: it is more likely after the age of 60, as soft tissues, such as cartilage and tendons, degenerate with the passage of time.

Prevention

It is not always possible to prevent accidental injuries, but there are ways to reduce the risk and protect the shoulder:

  • Listen to your body’s signals when performing an activity; if something hurts, lighten up until you recover.
  • Warm up before each activity, especially activities that involve throwing or hanging from the arms.
  • Maintain a healthy weight and activity level, as recommended by your physician.
  • If your doctor recommends it, do exercises to strengthen the muscles surrounding the shoulder to protect the joint.

Diagnosis of shoulder pain

Dr. Miguel Alfonso, an orthopedic surgeon specializing in shoulder pathologies and injuries, performs a thorough diagnosis.

  • To do this, he will initially evaluate your medical history and then perform a physical examination or evaluation.
  • In this physical evaluation, he will rule out any neck and back conditions. He will then evaluate a range of motion of the shoulder to determine the probable cause of your shoulder pain.
  • Dr. Alfonso Porcar may request some imaging tests to confirm the diagnosis of your shoulder pain, such as X-rays, ultrasound, MRI or CT.
  • If necessary, a shoulder arthroscopy may be performed to view the inside of your joint with a small camera.

Treatments for shoulder pain

There are a variety of treatments available for those experiencing shoulder pain.

Multidisciplinary pain management for complex cases may include:

  • Hyaluronic acid injections .
  • Prolotherapy injections for tendon and ligament injuries.
  • Platelet-rich plasma (PRP) infiltrations .
  • Other forms of regenerative medicine, using ultrasound to infiltrate in a guided manner at the exact point of injury.
  • Steroid injections in shoulder have also often been used with good success.
  • Surgical intervention is usually considered the treatment of last resort.

Platelet Rich Plasma Injections

PRP infiltrations are used in traumatology to stimulate the natural healing response that achieves the recovery of mobility, strength and normality of injured tissue.

This biological therapy induces the mechanisms for healing, and is used for the treatment ofrotator cuff tendinitis of the shoulder.

Platelet Rich Plasma (PRP) is also an effective alternative to corticosteroid injection therapy in the treatment of frozen shoulder.

Hyaluronic Acid Injections

Hyaluronic acid is a natural substance present in joint fluid that acts as a shock absorber and improves lubrication.

Viscosupplementation is a minimally invasive procedure, which involves the injection of a hyaluronic acid preparation into the shoulder to treat arthritis.

Indicated in patients with osteoarthritis or degenerative changes of the cartilage, delaying its evolution and even the need for a joint prosthesis.

Autologous Cytokine-Rich Serum

  • SARC contains cytokines, cell-released proteins and activators of the repair system.
  • These substances guide the cells towards the regeneration process.
  • Cytokines are a type of signaling molecules used in cellular communication and enhance healing in those cases where the tissue is in worse condition.
  • The cytokine-rich serum promotes decreased inflammation and activates cartilage repair.

Autologous serum rich in cytokines achieves up to 75% pain reduction for more than one year in patients with osteoarthritis.

No medication and no side effects.
