Orthopaedic Surgeon for Patients in Torrevieja
English-speaking orthopaedic and regenerative medicine care at Regenerat Elche, around 40 minutes from Torrevieja
Living on the Costa Blanca should mean walking the promenade, playing a round of golf or getting out on the padel court, not planning your week around a painful knee, hip or shoulder. If you have been told to “wait and see”, or that a joint replacement is your only option, a thorough specialist assessment in English can help you understand what is really going on and what your choices are.
We do not have a clinic in Torrevieja. Patients from Torrevieja, La Mata, Orihuela Costa and the surrounding urbanisations are seen at our clinic in the centre of Elche, where we carry out the consultation, the ultrasound scan and every treatment.
- Consultations in English, with reports and treatment plans explained clearly.
- No GP referral and no waiting list: you book directly with the specialist.
- Non-surgical options first, including PRP and other regenerative treatments.
Living in Torrevieja? Book your assessment in Elche
Tell us what is troubling you and send us any scans you have. We will find you an appointment and let you know what to bring.
We specialise in orthopaedics and regenerative medicine. From our clinic in Elche, we see patients from Torrevieja and across the Vega Baja and the southern Costa Blanca, including La Mata, Orihuela Costa, Guardamar del Segura, Rojales, Ciudad Quesada and San Miguel de Salinas.
Our approach differs from conventional orthopaedics: before talking about surgery or a joint replacement, we look at every non-surgical option, including the latest biological and regenerative treatments. Our aim is to get you walking, playing and moving again without pain, using the least invasive treatment that suits your particular case.
At Regenerat, your very first visit includes a clear, honest diagnosis, a full assessment with an ultrasound scan and a properly organised treatment plan, so the journey from Torrevieja is well worth it.

Private orthopaedic care that is worth the short drive
More than half of Torrevieja’s registered residents hold a foreign nationality, and many of them moved here to enjoy an active retirement. Getting a clear answer about a joint problem should not depend on how good your Spanish is, or on how long you can wait for a public hospital appointment.
Regenerat was founded by Dr Miguel Alfonso Porcar, a consultant orthopaedic surgeon with more than 20 years’ experience in osteoarthritis and regenerative medicine. Our approach is simple: find the real cause of your pain, explain it in plain English and offer the least invasive treatment that makes sense for you, keeping surgery for when it is genuinely needed.
We help you get rid of pain and get back to your daily activities and exercise as quickly as possible, using the safest and least invasive treatments available.
The problems Torrevieja residents most often bring to us
Whether you have lived here for twenty years or spend the winter months in the sun, these are the situations we see most often in patients travelling from Torrevieja and the south of the Costa Blanca.
Hip pain at night
Pain on the outside of the hip when lying on your side is often trochanteric bursitis or gluteal tendinopathy, not arthritis. Getting the diagnosis right changes the treatment completely.
Weak, painful hands
Struggling to open a jar or turn a key? Thumb base arthritis and carpal tunnel syndrome are very common from the mid-fifties onwards.
Golf, padel and bowls injuries
With some of the region’s best-known golf courses on the doorstep, we regularly treat golfer’s elbow, tennis elbow and rotator cuff problems that stop people playing.
“My knee won’t let me walk the promenade”
Knee osteoarthritis is the number one reason for a consultation. Stiffness in the morning, swelling after a walk or pain on the stairs are often the first signs.
Conditions we treat, joint by joint
Already have a diagnosis, or think you know what the problem is? Each link below explains the condition and its treatment options in English. All consultations take place at Regenerat Elche.
Knee
Worn cartilage, swelling and pain that limit walking or climbing stairs.
Hip
Groin pain, pain on the outside of the thigh or difficulty putting on shoes and socks.
Shoulder
Pain reaching up, getting dressed or swinging a club or racket.
Hand and wrist
Stiff fingers, pins and needles at night or a painful pinch grip.
Back
Lower back pain that makes standing or walking any distance difficult.
Elbow
Pain gripping, lifting or on impact with the ball.
Foot and ankle
Heel pain first thing in the morning or after a long beach walk.

Told you need a new knee or hip? Your options first
A joint replacement is sometimes the right answer, but not always the first one. Depending on how worn the joint is, there are often treatments that can ease pain and buy valuable years. This table gives an overview; your own plan is always agreed with you after a full assessment.
| Your situation | What we usually consider | Find out more |
|---|---|---|
| Mild to moderate osteoarthritis | Platelet-rich plasma (PRP), hyaluronic acid, or a combination of both, injected under ultrasound guidance | PRP · Hyaluronic acid |
| Osteoarthritis with bone marrow oedema and constant pain | Intraosseous PRP, which treats the bone just beneath the damaged cartilage | Intraosseous injections |
| Advanced knee osteoarthritis where injections no longer last | Monocyte therapy or, in selected patients, a long-lasting injectable hydrogel | Monocyte therapy · Arthrosamid® |
| Worn shoulder or hip tendons without a complete tear | Ultrasound-guided PRP or cytokine-rich serum; shockwave therapy for calcific deposits | Cytokine-rich serum · Shockwave therapy |
| Lower back pain from facet joint arthritis | Ultrasound-guided injections and radiofrequency ablation | Radiofrequency ablation |
| Very advanced arthritis with disabling pain | An honest surgical opinion: if a replacement is the best option, we will tell you | Our approach |
All injections are performed under ultrasound guidance at our Elche clinic, so the treatment reaches exactly the right spot.
How we organise your visit from Torrevieja
- Get in touch by phone or WhatsApp. Tell us what is troubling you and, if you have them, send us your scans or reports beforehand. English or Spanish reports, from Spain or from home, are all useful.
- Choose a convenient time. Mid-morning or early-afternoon appointments usually avoid the busiest traffic into Elche.
- A full assessment in one visit: medical history, examination and a diagnostic ultrasound, with the findings explained to you there and then.
- A plan that respects the distance: where possible, we start treatment on the day of your assessment and space out follow-ups.
- Stay in contact between sessions by phone or WhatsApp, so you only travel when it is really necessary.
Fees and insurance
First consultation: €100, including clinical assessment, examination, review of your scans and a diagnostic ultrasound.

We are an appointment-only clinic, not an emergency service. After a fall with severe pain, deformity or inability to bear weight, please go to A&E at your nearest hospital.
Questions from Torrevieja residents
Do you have a clinic in Torrevieja?
No. Patients from Torrevieja, La Mata, Orihuela Costa and nearby are seen at Regenerat Elche, C/ Almòrida 8, in the centre of Elche. The consultation, ultrasound and all treatments take place there.
Will my consultation be in English?
Yes. We can see you in English at our Elche clinic, and we take the time to explain your diagnosis and treatment options clearly. You are welcome to bring a family member or friend with you.
How long does it take to get to Elche from Torrevieja?
It is about 45 km by car, roughly 40 minutes via the CV-905 and the AP-7. There is also an intercity bus to Elche bus station, which takes around an hour; from there it is a short taxi or local bus ride to the clinic.
Do I need a referral from my GP or medical centre?
No. You can book directly with the specialist by phone, WhatsApp or through our contact form, with no referral needed.
Can I use my S1, EHIC or GHIC card?
No. S1 forms and EHIC or GHIC cards give access to Spain's public health system, and Regenerat is a private clinic. Consultations are paid privately or through a private health insurance policy that covers them.
Do you accept private health insurance?
Yes, depending on the agreement with each insurer, including some international insurers. Please call us with your policy details before your appointment and we will confirm your cover. We also see self-paying patients.
Can I bring scans and reports from the UK or from Torrevieja Hospital?
Yes, please do. Recent X-rays, MRI scans and reports, in English or Spanish, help us reach an accurate diagnosis and avoid repeating tests. If possible, send them to us before your visit.
I've been told I need a knee or hip replacement. Can I get a second opinion?
Yes. This is one of the most common reasons people come to see us. We review your scans, examine you and explain whether there are reasonable alternatives before surgery. If a replacement is the best option for you, we will tell you so honestly.
I only spend the winter in Spain. Can I finish treatment before I go home?
In most cases, yes. Tell us your travel dates when you book and we will plan your assessment and sessions around them. A typical PRP course for the knee is three sessions two to four weeks apart, so it usually fits within a winter stay.
How much does the first consultation cost?
The first consultation costs €100 and includes a clinical assessment, examination, review of your scans and a diagnostic ultrasound. Follow-up consultations cost €80.
