What is hip bursitis?
Hip bursitis is caused by inflammation of the small synovial fluid-filled sacs called bursae or synovial bursae around the hip joint.
The bursae, which provide cushioning between the soft tissues (muscles, tendons and ligaments) and the hip bones.
Most commonly, bursitis causes pain in the hip joint that extends to the outside of the thigh area.
Bursitis on the outer (lateral) side of the hip is called greater trochanteric bursitis or hip bursitis. It occurs when the bursa on the outside of the hip becomes damaged, irritated or inflamed.
There are several types of hip bursitis depending on the bursa or bursa affected.
Trochanteric Bursitis – Trochanteric Bursitis
The trochanteric bursa is located on the outside of the hip, over the prominent bone called the greater trochanter, which is located at the top of the femur.
- Symptoms of trochanteric bursitis: Pain in the outer hip, thigh and buttocks, which may worsen with activity, climbing stairs or lying on the side. There may also be tenderness to touch and swelling in the affected area.
- Causes: Repetitive motions, such as running, stair climbing or impact sports; direct trauma to the hip; muscle weakness; hip imbalance; inflammatory diseases such as rheumatoid arthritis.
Iliopsoas bursitis or ischial bursitis
This bursitis occurs when the synovial bursa of the iliopsoas muscle, located in the front of the hip near the groin, becomes inflamed. Symptoms include pain in the groin area and in the back of the hip that may radiate to the thigh or lower abdomen.
- Symptoms of ischial bursitis: Pain in the groin, inner thigh and lower back, which may be worse when the hip is flexed or rotated inward. Stiffness and difficulty walking may also be present.
- Causes: Repetitive motions involving hip flexion, such as bicycling, swimming or golfing; direct trauma to the groin; muscle weakness; hip imbalance; inflammatory diseases such as rheumatoid arthritis.
Septic hip bursitis
Septic hip bursitis is an inflammation of the synovial bursa of the hip caused by infection. This condition occurs when bacteria or other microorganisms invade the synovial bursa, causing inflammation and pain. It requires urgent medical attention.
Causes of hip bursitis
An irritated and inflamed bursa causes pain that can limit range of motion and make some activities or positions uncomfortable.
Certain positions, movements or diseases can cause friction or stress on the bursa and lead to bursitis or other hip injuries. When the bursa is injured, the muscles and tendons do not glide smoothly, which can cause pain.
Hip bursitis is most common in women and middle-aged people, and develops as a result of repeated irritation to the hip. It is rarely the result of a single injury.
Hip bursitis can be caused by:
- Repeated movements that irritate the bursa such as walking, running on hills or uneven surfaces, or climbing stairs, standing for long periods….
- Abnormal walking due to injury, leg length differences or spinal problems.
- Muscle weakness.
- Incorrect posture
- Hip surgery or replacement.
- Bone spurs in the hip.
- Infections and autoimmune diseases, such as rheumatoid arthritis, gout, psoriasis or thyroid disease; in this case it is called septic bursitis.
- Direct trauma from blows or falls on the side of the hip can irritate the synovial bursa causing trochanteric bursitis.
Trochanteric bursitis is the main form of hip bursitis.
Hip bursitis symptoms
In hip bursitis, the main symptom you may experience is pain in the outer hip, thigh, buttock or outer leg.
That pain may be worse when you get up after sitting, for example, from a low chair or car seat.
Pain can also be triggered by:
- Push with your fingers on the external part of the hip.
- Lie on the affected hip.
- Climbing stairs.
- Walking or running.
- Lift the leg to one side.
- Sitting cross-legged.
Diagnosis of hip bursitis
To diagnose bursitis, Dr. Miguel Porcar will review your symptoms, medical history and perform a complete physical examination, focusing on how long you have had the condition and the specific movements that are causing your pain.
Other tests for diagnosis may include:
- X-rays for hip bursitis: X-rays do not show the soft tissues such as the bursae of the hip and are therefore usually normal. However, they are requested to rule out other problems.
- MRI and ultrasound: create better images of the soft tissues and may show fluid or inflammation in the bursa.
- In some cases, we will inject the bursa with an anesthetic. If the pain goes away immediately, you most likely have bursitis.
Treatments for hip bursitis with injections
Treatment for hip bursitis begins with rest measures , ice, analgesics and activity modification.
Other conservative treatments include:
- Simple lifestyle modifications and changes, such as avoiding activities or sports that cause hip pain and impact.
- Nonsteroidal anti-inflammatory drugs such as ibuprofen and naproxen reduce pain and inflammation.
- Physical therapy for the treatment of bursitis usually involves exercises to strengthen the muscles, as well as stretching and avoiding activities that cause pain. This helps prevent muscle atrophy and can also be used to prevent bursitis, not just treat it.
If these treatments do not resolve the bursitis, it may be necessary to remove fluid from the bursa.
Do injections help hip bursitis?
Yes, injections can effectively treat hip bursitis.
- In some cases the doctor may recommend steroid injections to relieve pain and swelling. Corticosteroid injections are commonly used to reduce inflammation and alleviate pain in the affected bursa. These injections provide relief by decreasing swelling and discomfort.
- Ultrasound-guided injections with platelet-rich plasma are very effective and delay or even avoid surgery.
Surgery is rarely necessary to treat bursitis and is only considered when all other treatments have failed.
How long does a hip bursa injection last?
Corticosteroid injections can relieve hip bursitis pain for weeks to six months. PRP injections may take longer to show effect but often provide longer-lasting relief—up to 12 months—by promoting tissue healing. The choice depends on symptoms, goals, and medical advice.
Treatment for hip bursitis with corticosteroid injection
Using ultrasound-guided techniques, we can administer an injection of local anesthetic and a cortisone preparation.
Cortisone is a very effective anti-inflammatory drug, and injecting it into the bursa relieves pain and reduces inflammation.
Steroid injections are usually very effective in treating bursitis.
You may need another injection after a few months.
