What is ankle osteoarthritis?
Osteoarthritis of the ankle is an inflammation resulting from degeneration of the joint cartilage that causes pain, swelling and stiffness in the joints, resulting in limited movement.
It may occur due to fracture, dislocation, inflammatory disease or congenital deformity.
The joints of the foot most commonly affected by osteoarthritis are:
- The joint between the shin bone (tibia) and the ankle bone (talus).
- The three joints of the foot that include the heel bone, the inner medial foot bone and the outer medial foot bone.
- The big toe joint and the bone of the foot
Causes of ankle osteoarthritis
There are three types of arthritis that affect the foot and ankle and they can be:
- Osteoarthritis is a degenerative joint disease that occurs more frequently in older people. This disease affects the cartilage, the tissue that cushions and protects the ends of the bones in a joint.
The cartilage begins to wear away and in extreme cases can wear away completely, leaving nothing to protect the bones of a joint, causing bone-on-bone contact.
Bones may also bulge or protrude at the end of a joint, which is called a bone spur.
- Post-traumatic ankle osteoarthritis: Ankle osteoarthritis most often occurs after a major traumatic injury to the ankle.
A pylon fracture can cause osteoarthritis of the tibiotalar (ankle) joint; a displaced fracture of the calcaneus can cause subtalar osteoarthritis. Osteoarthritis is also seen after less severe injuries, especially if those injuries cause malalignment.
This condition can develop years after the trauma, such as a fracture, severe sprain or torn ligament.
- Rheumatoid arthritis: an autoimmune disease in which the body’ s immune system (the body’s way of fighting infection) attacks healthy joints, tissues and organs. It can cause pain, stiffness, swelling and loss of function in the joints.
Symptoms of osteoarthritis of ankle
Symptoms of arthrosis of the foot and ankle include pain or tenderness, swelling and stiffness in the joint, and limited range of motion.
It is important to determine the exact nature, location, duration and progression of symptoms in order to narrow down the type of osteoarthritis and the structures affected.
The presentation in patients varies according to the type and location of osteoarthritis.
- Patients with hindfoot osteoarthritis present with pain, stiffness and swelling. Symptoms are usually aggravated by activity and relieved by rest.
- In the case of ankle osteoarthritis, the pain usually appears on the anterior aspect of the ankle joint, in the form of a band along the tibiotalar joint.
- In subtalar osteoarthritis, the pain is usually localized to the lateral aspect of the hindfoot, often below the fibula in the tarsal sinus, although sometimes the pain also radiates medially.
Identification of aggravating and mitigating factors can also help identify the location of osteoarthritis.
Diagnosis of osteoarthritis of the ankle
For the diagnosis Dr. Miguel Alfonso Porcar will start from the patient’s medical history, to know if there is a history of disease or injury, and to rule out other diseases that may be the cause of the discomfort.
A clinical examination and functional mobility and stability tests to check mobility and pain reactions using manual maneuvers directed by the traumatologist.
We will also use imaging techniques to examine the condition of the ankle.
X-rays can clearly show significant wear damage. Special attention is paid to the size of the joint space. The smaller it is, the more advanced the osteoarthritis.
Treatment of ankle osteoarthritis
There are several treatment options for osteoarthritis of the ankle.
Cartilage loosening of the ankle joint cartilage can be avoided conservatively by non-impact strengthening exercises and physical therapy.
Conservative treatment of osteoarthritis of the ankle
- This mainly includes exercises to strengthen the protective and stabilizing muscles of the ankle.
- Unloading insoles can also be used, as well as the use of ankle braces.
- Analgesics and anti-inflammatory drugs can be used to relieve pain and prevent inflammation.
Hyaluronic acid injections for ankle osteoarthritis
Intra-articular injection of hyaluronic acid restores the properties of the synovial fluid protecting the arthritic joint from deterioration.
Its use is indicated in patients with osteoarthritis or degenerative changes of the cartilage, delaying its evolution and even the need for a joint prosthesis.
Treatment of ankle osteoarthritis with serum rich in cytokines
Treatment of ankle osteoarthritis with autologous serum rich in cytokines (SARC), such as QREM Cytokine, is an innovative option in regenerative medicine.
This approach uses serum obtained from the patient’s own blood, enriched with cytokines and growth factors, especially the interleukin-1 receptor antagonist (IL-1Ra), which has anti-inflammatory properties.
Treatment procedure
- Blood collection: Approximately 40 ml of blood is drawn from the patient.
- Processing: The blood is processed in a specialized device, such as the QREM Cytokine, for about 37 minutes, obtaining a cytokine-enriched serum.
- Infiltration: The serum obtained is injected directly into the affected ankle joint, usually under ultrasound guidance to ensure accuracy.
Benefits of cytokine-rich serum for the treatment of osteoarthritis of the ankle
- Pain reduction: A pain reduction of up to 75% has been observed for more than one year in patients with osteoarthritis.
- Decreased inflammation: Anti-inflammatory cytokines present in the serum help reduce joint inflammation.

PRP – Platelet Rich Plasma Injection
Intra-articular autologous PRP injections are more effective than hyaluronic acid injections in reducing pain and restoring joint function, especially in younger patients.
Due to its anti-inflammatory and analgesic effect, PRP reduces joint stiffness and slows down the progression of joint degeneration.
Surgery for osteoarthritis of the ankle
If the amount of cartilage in the ankle joint has already decreased significantly, surgery is another way to slow the progression and counteract its symptoms.
Surgical treatments are complex and should only be considered if conservative and regenerative therapies such as ankle infiltrations have not been successful and the pain is persistent and very intense.
- Arthroscopic debridement surgery: outpatient procedure with rapid recovery (4-6 weeks). Suitable for early osteoarthritis. Arthritic spurs and loose bodies can be removed, and irregularities in the remaining cartilage can be scraped and smoothed.
However, because the underlying arthritic process is still present, both the rate of improvement and the length of time the benefit lasts are less than for major surgeries.
- Ankle joint fusion or arthrodesis: consists of removing bone from the tibia and talus and joining them with screws. Over time, the ends of the bones grow or fuse together. Although ankle motion is eliminated, the adjacent joints compensate.
The primary role of fusion is in young patients with increased physical demands and where the ankle is the only joint affected. It is also the best option in those with a malaligned ankle.
In these circumstances, a successful fusion is very reliable in providing long-term pain relief that results in a limp-free gait and allows a return to more physical work.
The best option for each patient depends on many factors, such as the severity of osteoarthritis, the patient’s age and functional requirements.
The final choice is a joint decision between the surgeon and the patient.
