Shoulder tendonitis: What is supraespinatus tendinopathy?
The rotator cuff consists of a group of tendons and muscles that surround and stabilize the shoulder joint.
These tendons allow a wide range of motion of the shoulder joint in multiple planes. Irritation or injury to these tendons can cause rotator cuff pain.
Tenderness in the front of the shoulder or at the midpoint of the arm is usually caused by rotator cuff pain.
What is supraespinatus tendinopathy?
Supraspinatus tendinopathy is a common shoulder condition characterized by damage or degeneration of the supraspinatus tendon, a key component of the rotator cuff.
This tendon facilitates arm elevation and stabilization. Overuse, repetitive overhead activities, or aging can lead to tendinopathy, resulting in shoulder pain and reduced function.
When lifting your arms, you may experience pain and stiffness in your shoulder. Symptoms of night pain and difficulty sleeping are also problems associated with supraespinatus tendinopathy and rotator cuff pain.
We must think that the place where the set of tendons of the rotator cuff coincide is limited. This fact has a consequence, the irritation, and, therefore, the increase in size of one of them, is going to harm the rest.
This can be a vicious circle if a proper diagnosis and treatment is not carried out, causing limitations in the patient’s daily life.
Causes of supraespinatus tendonitis
Rotator cuff pain may be due to:
- Age-related wear and tear (overuse, overexertion, etc.).
- Repeated movements of the arm above the head, (typical of manual workers).
- Sports activities that require the throwing of objects or that require great effort of the arms above shoulder height.
- Trauma such as accidents, direct blows to the shoulder or falls on the shoulder.
Symptoms of rotator cuff tendonitis
Symptoms that indicate you may have a rotator cuff tendon injury in the shoulder are:
- Shoulder weakness.
- Pain radiating down the side of the arm from the front of the shoulder.
- Sudden pain when performing throwing exercises or strength exercises against gravity by lifting objects above shoulder height.
- Restriction of range of motion.
Among the main factors that aggravate rotator cuff pain are performing overhead arm movements, arm movements to touch the back, lifting objects, lying on the shoulder.
Onset symptoms may be mild and patients often do not seek treatment at an early stage.
As the problem worsens, symptoms may increase to loss of strength and difficulty sleeping at night among others.
Prevention of supraespinatus injuries
Rotator cuff pain can be prevented by taking the following precautions:
- Perform regular shoulder stretching and strengthening exercises.
- Rest from time to time while performing repetitive activities with overhead arm movements.
- When carrying a heavy load (e.g. a shopping bag), change sides frequently.
Why won’t my shoulder tendonitis go away?
Persistent shoulder tendonitis can result from several factors:
- Continued overuse: Engaging in repetitive overhead activities without adequate rest can perpetuate inflammation.
- Incomplete rehabilitation: Insufficient or improper rehabilitation may fail to restore full tendon strength and flexibility, leading to ongoing symptoms.
- Structural abnormalities: Bone spurs or anatomical variations can impinge on tendons, hindering healing.
- Underlying conditions: Medical issues like diabetes or thyroid disorders can impair tendon healing.
Diagnosis of tendinitis of the shoulder

- In the physical examination we will perform a series of selected movements to evaluate if your shoulder injury is compatible with the rotator cuff, or if it is related to neighboring structures of the body.
- We may perform an X-ray or ultrasound of the shoulder to make sure that there are no problems affecting the shoulder structures.
- Ultrasound is a very valid test for diagnosis, but even more so for treatment. In the clinic its use is available to perform guided infiltration in the exact point of greatest pain and/or injury.
- Finally, Magnetic Resonance Imaging will be a diagnostic test to be used, performing a broader study of the tissues surrounding the joint and ruling out neighboring lesions.
Treatment options for Supraspinatus Tendonitis and Rotator Cuff Tendinitis

How do we treat chronic supraspinatus tendonitis?
Initial treatment is conservative in most cases based on rest, non-steroidal drugs, physiotherapy and/or infiltrations. Many patients experience gradual improvement and return to normal activity, although this option may take several weeks to months.
For chronic supraspinatus tendinopathy, prolotherapy with dextrose, steroid and PRP injections are the common treatment options
- Steroid injections: Provide quick pain relief by reducing inflammation but may weaken tendons over time with repeated use. Effects typically last weeks to months.
- PRP (Platelet-Rich Plasma) Injections: Use the patient’s own platelets to promote tendon healing. Results take longer but may offer more sustained improvement (more than 1 year) without tendon damage.
Steroid injection for supraspinatus tendonitis
With ultrasound-guided infiltration, we can administer an injection of local anesthetic and a corticosteroid preparation into the affected tendon bursa.
Corticosteroids are a safe medication in most patients. This drug provides temporary pain relief and allows for increased joint function.

Once the lesion has been studied, we apply the injection in the exact or closest point, in order to provide the patient with the greatest benefit.
Using ultrasound-guided ultrasound, an injection of corticosteroids can be administered directly into the sheath of the affected tendon(s). This helps to relieve pain and perform strength exercises without any pain.
Platelet Rich Plasma in the Shoulder
This is an effective pre-surgical procedure that uses a concentrated dose of platelets prepared from your own blood.
Prior to the PRP injection in the supraespinatus tendon, the patient undergoes a blood extraction. After a period of processing, a substance is obtained that has proven to have reparative and regenerative capacity known as Platelet Rich Plasma.

Shoulder surgery is indicated when the patient does not experience improvement after the use of non-surgical treatments.
Surgical intervention can be performed by open or arthroscopic surgery.
The surgical procedure consists of the exploration and treatment of the lesions in relation to the rotator cuff tendons, as well as studying or ruling out other associated lesions.
In this way we recover the function of the rotator cuff and allow unrestricted movement of the joint.
Through two or three tiny incisions in the shoulder, a camera is introduced inside the shoulder to explore the injured joint. Through the remaining incisions, the procedure is performed, thus avoiding a large scar and further patient recovery.
After surgery we will place your arm in a sling for a short period of time for a faster recovery. And as soon as possible we will remove the sling to start exercising and strengthening the muscles.
