What is prolotherapy?

Prolotherapy, or proliferative therapy, is a form of regenerative medicine used to treat pain. It involves injecting various substances into different structures of the musculoskeletal system to alleviate discomfort.

These substances promote an inflammatory response, initiating the regeneration of injured tissues.

Proliferative therapy is a precise pain relief technique in which damaged or problematic tissues—especially tendons, ligaments, joints, menisci, and nerves—are deliberately “disrupted.”

What does prolotherapy consist of?

Dr. Miguel Alfonso Porcar will inject a small amount of an irritant agent, usually dextrose, into the area to be treated.

Through proliferative therapy, the dextrose solution, which serves as the primary proliferant, triggers an inflammatory reaction process that irritates the tissue where it has been injected, prompting the patient’s body to initiate its own repair mechanisms.

The mechanism of action is based on creating a local inflammatory process that stimulates collagen production in the injured tissues, thereby strengthening the structure of cartilage, ligaments, and tendons through fibrosis.

Sensing the minor damage caused by the injection and the subsequent irritation in the injected area, the body sends healing nutrients to the site and begins to generate new tissue.

The therapeutic approach varies according to the severity and type of injury. Based on these factors, either simple prolotherapy sessions can be considered or a more complex treatment known as Integral Prolotherapy may be recommended.

The latter may include combining dextrose injections with platelet-rich plasma (PRP).

Stretched, worn, or torn ligaments and tendons can be revitalised with this technique.

Why is prolotherapy used?

Prolotherapy is an alternative to surgery for treating chronic pain.

Many bodily pains, particularly back and spinal pain, are often “referred” pains. Although the pain may be felt at a specific point on the patient’s body, the actual source of the pain may be located far from that point.

An easy example of this is arm pain, which is actually caused higher up in the body by a pinched nerve in the neck.

Even in the best circumstances, surgery is a serious consideration. Although most modern surgical techniques are well-proven, they are not always the appropriate first step for a particular problem.

Modern diagnostic imaging techniques, however powerful they may be, cannot always pinpoint the exact origin of joint or back pain with 100% certainty.

The body structures treated by prolotherapy are often hidden, difficult to visualise, or too worn for surgical repair.

By reconstructing tissue directly in these problematic areas, this therapy can often repair connective tissue damage to ligaments and tendons that surgery cannot address.

What is treated with prolotherapy?

Prolotherapy is used for chronic pain caused by conditions of the musculoskeletal system.

It can reduce pain associated with various diseases or disorders affecting bones and tissues, including ligaments, muscles, tendons, and cartilage.

Prolotherapy is beneficial for individuals with the following conditions:

  • Arthritis.
  • Arthrosis.
  • Ligament instability.
  • Fibromyalgia or chronic pain.
  • Degenerative disc disease.
  • Tendinopathies.
  • Low back pain.

 

What is prolotherapy for?

Prolotherapy is an effective treatment for triggering the body’s natural healing responses.

The injection of a prolotherapy solution based on hypertonic dextrose (sugar water), applied directly into the connective tissue ligaments of the joints, creates localized inflammation by irritating the injected area.

The localized irritation that sets in motion the proliferation and regeneration of the injured tissues through the body’s natural healing mechanisms, initiates the process of new tissue formation in the injected area.

In essence, the therapy tricks the body into repairing a particular area, causing the cells in that area to kick in and heal in a way that they did not before.

The body responds to the dextrose from the prolotherapy injections as a threat or injury, triggering immune and healing responses that go to the area to “repair” the dextrose and, in the process, repair the damaged tissue.

What results are achieved with prolotherapy?

After the necessary prolotherapy sessions according to the patient’s pathology, improvements are achieved in:

  • Functionality: since prolotherapy increases the patient’s ability to perform activities of daily living such as climbing stairs.
  • Strength: Damage to ligaments and tendons can result in decreased muscle strength. A notable result of Prolotherapy is improved strength in painful areas as pain is reduced.
  • Pain: Pain relief is one of the most common reasons patients seek Prolotherapy. Therefore, a significant decrease in pain is indicative of the success of Prolotherapy.
  • Stiffness: Patients often experience stiffness when getting up in the morning or after certain activities. Prolotherapy, by helping to stabilize the treated area, allows the muscles to stop providing such stability and relax more easily, thus decreasing stiffness.

Ultrasound, unlike other imaging tests such as X-rays and MRI, is extremely useful to monitor these types of lesions and also to perform the injection of prolotherapy solutions with greater precision through ultrasound-guided procedures.

Knee prolotherapy for the treatment of tendinopathies

Tendinopathies with pain derived from tendon disorders, such as tendinitis.

Use for chronic, painful knee, shoulder or hip tendon conditions resulting from age or overuse of one or more joints showed 30-40% improvements in tendon strength.

Clinical data conclusively demonstrated that the therapy is an extremely viable treatment for tendon and ligament problems in patients whose conditions do not improve with conventional therapies such as physical therapy or cortisone injections.

It is mainly used in cases of distension or elongation. When there is alteration of the structure or breakage of the structure, platelet-rich plasma or the combination of prp plus prolotherapy is recommended.

Prolotherapy Injections in the Knee: Treatment of Osteoarthrosis and Related Pathologies

Osteoarthritis is a disease characterized by cartilage degradation and alteration of the natural collagen in the joints.

Prolotherapy produces significantly greater improvement in the reduction of chronic pain, inflammation, buckling episodes and flexion range compared to analgesic injections or exercise.

Patients who had received the glucose injections showed significantly greater improvement one year after treatment than control patients who did not receive them.

The majority of study participants were still experiencing improvement in knee pain, function and stiffness scores even more than two years after the start of the study.

Spinal/pelvic treatment – Lumbar prolotherapy: Pain in the back and hips

In 90% of patients, low back pain is usually caused by overuse, strain, lifting or bending. Resulting damage includes sprained ligaments, pulled muscles and herniated discs.

Everyone knows that a herniated disc is painful, but it is important to remember that it is the ligaments that hold the vertebral discs in place. When the ligaments are weak, the disc is more likely to herniate, or “slip”.

Prolotherapy produces greater long-term pain reduction than the typical corticosteroid injection for patients with back pain.

These results are valid for patients with chronic low back pain, pelvic pain and cervical pain due to traffic accidents.

Treatment of muscle pain with proliferative therapies (prolotherapy)

Muscle pain is usually due to a state of insufficient energy metabolism in the muscle.

Therapeutic injections into myofascial (muscle) trigger points are designed to stimulate energy production at the cellular level. Increased activity results in increased metabolism at the trouble spot and a possible end to pain.

Prolotherapy patients obtain improvements in pain and perceived pressure.

 

What to do before prolotherapy?

Talk to Dr. Alfonso Porcar before starting prolotherapy. He will tell you if it is necessary to pause any medications you are taking or other pain control techniques you are using.

Before proceeding with prolotherapy, it is essential to perform a detailed examination of the patient’s conditions.

  • This involves an accurate clinical and imaging diagnosis of the source of the pain. It is also crucial to determine the exact location for the injection of the substance, which sometimes requires the assistance of ultrasound guidance.
  • It is also essential to consider that, in parallel to prolotherapy, those biomechanical factors that contributed to the deterioration of the joint, tendon or ligament, such as overweight, inadequate posture, poor sports technique, postural imbalances, among others, should be adjusted.
  • For complex cases of osteoarthritis of the knee, we can use a prolotherapy-only approach, or we can combine it with PRP (platelet-rich plasma) treatment, or autologous cytokine-rich serum.

What happens during prolotherapy sessions?

In the same session, once we have your diagnosis, we will inject a solution containing hypertonic dextrose (or other solution) into the joints, tendons or ligaments where you feel pain.

We may use an imaging tool, such as ultrasound, to guide the injections.

How many prolotherapy sessions do I need?

Depending on the type of injury you will need to receive prolotherapy in several injection sessions. Typically three to six injections over the course of all prolotherapy sessions. The number and frequency of injections will depend on the area of the body being treated and what your doctor recommends.

Large, complex joints such as the knee and shoulder usually require a series of five injections, spaced over time, to achieve a full healing effect.

Smaller joints, such as elbows or ankles, may need only one or two injections.

What care is necessary after prolotherapy?

Most of the time, prolotherapy does not require any special follow-up.

Due to the irritation of the treated area generated by the prolotherapy, it may happen that after the injection the pain worsens. If this is your case we can give you analgesics to treat the pain.

This pain is a normal reaction and usually subsides within a few days.

What are the advantages of prolotherapy?

The advantages of prolotherapy are:

  • Pain reduction: many people report a marked reduction in pain after injections.
  • Very few side effects: it is a low-risk treatment that does not usually have serious side effects.
  • It can be combined with other treatments: we can use prolotherapy in addition to other treatments and techniques to control your pain.

What are the risks of prolotherapy?

Fortunately, prolotherapy has no serious side effects.

You may feel some pain at the injection site and bruising is rare.

There is a very low risk that the injections will damage nerves, joints or other tissues near the injection site, but prolotherapy is no more risky than any other injection.

The risks most associated with prolotherapy have more to do with its efficacy than with physical damage to the body.

How much does a prolotherapy session cost?

Most insurance plans do not cover prolotherapy injections, which means you will have to pay the full cost out of pocket.

The cost may vary depending on your city and the clinic, but you may have to pay from 100 to 150 € per prolotherapy session. In our clinic the price per session is 75 €.

What is the recovery time after prolotherapy?

Prolotherapy sessions require little recovery time.

You should be able to resume all your normal activities immediately. You may have some soreness at the injection site, but you should be able to resume all of your normal activities without interruption.

Talk to your doctor before resuming strenuous physical activities, such as strenuous workouts or playing sports, after an injection.
