What is a shoulder rotator cuff tear?
The rotator cuff is the group of 4 muscle tendons that maintain the stability and overall function of the shoulder.
Supraspinatus tendon: This tendon is located at the top of the shoulder and is responsible for lifting and rotating the arm outward.
Infraspinatus tendon: It is located in the back of the shoulder and is essential for the external rotation of the arm.
Tendon of the teres minor: It is also located in the back of the shoulder, just below the infraspinatus, and helps in the external rotation of the arm.
Subscapularis tendon: This tendon is located at the front of the shoulder and is essential for internal rotation of the arm.
Damage to the rotator cuff can limit the mobility and strength of the affected arm.
The two main forms of rotator cuff injury are:
- Partial or complete rupture of the supraspinatus tendon or other rotator cuff tendon. or other rotator cuff tendon: occurs when at least one of the muscles or tendons that make up the rotator cuff is injured or torn.
The tear may be acute (following a sudden injury or fall) or chronic (aggravation of the muscle or tendon over time). Tears can be partial or total.
It is typical in two types of patients: young athletes who routinely overexert the joint; and middle-aged or elderly people due to degeneration or occupational overuse.
- Tendinopathies: this is a group of diseases affecting the rotator cuff tendons.
In this case there is no partial or total tendon rupture, but an inflammation or degeneration is generated that prevents the normal function of the joint.
Causes of rotator cuff injury or tear in the shoulder
Rotator cuff pain may be due to:
- Age-related wear and tear (overuse, overexertion, etc.).
- Repeated movements of the arm above the head, (typical of manual workers).
- Sports activities that require the throwing of objects or that require great effort of the arms above shoulder height.
- Trauma such as accidents, direct blows to the shoulder or falls on the shoulder.
What are the symptoms of a rotator cuff tear?
A rupture of one or more rotator cuff tendons causes severe pain, weakness of the arm and a sensation of crepitus when moving the shoulder in certain positions.
After the acute or most clinical episode, it causes stiffness, swelling, limited range of motion and tenderness in the anterolateral aspect of the shoulder.
It is a pathology that we must give importance. In a first period we can think that it is something temporary, but if we do not make a correct diagnosis and treatment, the capacity of movement and the necessary strength for the activities of the day to day will diminish.
The most common symptoms are:
- Pain when sleeping on the affected area. Inability to rest at night.
- Dull pain in the shoulder joint or upper shoulder.
- Gradual increase in pain during physical activity.
- Loss of muscle strength due to decreased arm mobility.
- Inability to perform daily activities: pain when combing your hair, lifting your arm or stretching your hand behind your back…
What are the causes of a rotator cuff injury?
You may be more at risk for a rotator cuff injury if you work in an industry that involves a lot of repetitive motions, such as heavy lifting.
The same is true for sports with a large number of repetitive actions, such as archery, weightlifting or tennis.
If you have diabetes, you may be at an increased risk for rotator cuff injury, as your tendons may be affected by weaker circulation in the body.
Muscle differences in the shoulder can also cause shoulder pain and dysfunction.
As you get older, you are more likely to suffer a rotator cuff injury. The same is true if you are overweight, or a family member has had a rotator cuff injury in the past.
A lesion may also appear due to calcification calcification or calcium deposit in the tendons in what is called calcifying tendinitis.
Diagnosis of rotator cuff injury
Dr. Porcar will evaluate your condition based on your symptoms and physical examination.
- The physical examination will ask you to perform a series of selected shoulder movements to assess whether your injury is compatible with the rotator cuff or related to neighboring structures of the body.
- We may perform an x-ray or shoulder ultrasound to make sure there are no problems affecting the shoulder structures.
- Ultrasound is a very valid test for diagnosis, but even more so for treatment. In the clinic its use is available to perform guided infiltration in the exact point of greatest pain and/or injury.
- Finally, Magnetic Resonance Imaging will be a diagnostic test to be used, performing a broader study of the tissues surrounding the joint and ruling out neighboring lesions.
Rotator cuff tears with non-surgical treatments
The treatment plan for a rotator cuff injury depends on the severity of the condition.
Mild rotator cuff injuries can be treated with ice, rest and physical therapy during the acute phase to obtain pain relief and initiate early mobility to stop the loss of muscle strength.
For more severe rotator cuff injuries, treatment may include:
- In-clinic and home exercises designed to restore the function of the affected joint. All of them individualized according to the type of patient and his injury.
- Steroid injections in the shoulder.
- Injections of Platelet Rich Plasma or Cytokine Rich Serum.
In severe cases of injury with complete cuff rupture, surgical intervention may be recommended when there is no response to previous treatments.
Treatment of rotator cuff injuries with steroid injections
With ultrasound by means of an ultrasound-guided technique, we administer an injection of local anesthesia and a corticoid preparation in the bursa of the affected tendon (never intra-tendon; for this reason it is very important to perform the infiltration with ultrasound control).
Corticosteroids are a safe medication in most patients. This drug provides temporary pain relief and allows for increased joint function.

Once the lesion has been studied, the injection is applied in the exact or closest point, with the objective of providing the patient with the greatest benefit.
Rotator cuff tear treatment with Platelet Rich Plasma
This is an effective pre-surgical procedure that uses a concentrated dose of platelets prepared from your own blood.
Prior to the injection, the patient undergoes a blood extraction. After a period of processing, a substance is obtained that has proven to have reparative and regenerative capacity known as Platelet Rich Plasma.
It is a safe technique, since the substance is derived from the patient himself, so it is very difficult for there to be adverse effects after the infiltration. The clinic ensures that the processing of the sample is optimal and then the infiltration is performed with the help of ultrasound.

Rotator cuff tear surgery
Shoulder surgery is indicated when the patient does not experience improvement after the use of non-surgical treatments.
Surgical intervention can be performed by open or arthroscopic surgery.
The surgical procedure consists of repairing partial or complete rotator cuff tears, as well as studying or ruling out other associated injuries.
In this way we recover the function of the rotator cuff and allow unrestricted movement of the joint.
Through two or three tiny incisions in the shoulder, a camera is introduced inside the shoulder to explore the injured joint. Through the remaining incisions, the procedure is performed, thus avoiding a large scar and further patient recovery.
After surgery we will place your arm in a sling for a short period of time for a faster recovery. And as soon as possible we will remove the sling to start exercising and strengthening the muscles.

Causes of shoulder pain that shoulder surgeons can treat
