What is sacroiliitis (si joint pain)?
Sacroiliitis is the acute inflammation of the sacroiliac joints, it can be unilateral or bilateral. It is frequently associated with rheumatic processes.
The sacroiliac joint connects the spine and pelvis between the sacrum and the ilium. It is a rigid joint with very little mobility.
What is bilateral sacroiliitis?
Bilateral sacroiliitis is a simultaneous inflammation of both sacroiliac joints. These joints are located in the lower back, where the sacrum (the triangular bone at the base of the spine) meets the iliac bones of the pelvis.
What is sacroiliac joint pain?
SI joint pain is a degenerative wear and tear of the cartilage and joint surface. The result can be pain that can lead to movement restrictions in the back and hip area . SI joint problems can also become chronic.
Osteoarthritis of the joint may be the cause of pain in the buttocks, side of the hip and lower back. Those affected may feel this pain when getting out of the car, standing for a long time or walking slowly.
The disease is degenerative. This means that the damage is permanent and the wear and tear cannot be repaired. However, its progression can be positively influenced with proper treatment.
Which specialist treats bilateral or unilateral sacroiliitis?
Sacroiliitis is an inflammation of one or both sacroiliac joints, which can result from a variety of causes, including injury, arthritis, infection, or even pregnancy.
Given the complex nature of this condition, several specialists may be involved in its diagnosis and treatment.
Specialists treating sacroiliitis: Traumatologists and Physiotherapists
Traumatologists specializing in the treatment of sacroiliitis can offer treatments ranging from the most conservative treatment, through regenerative medicine treatments such as prolotherapy, PRP infiltrations for sacroiliac osteoarthritis, to surgical interventions if necessary.
Other specialists who can treat sacroiliac osteoarthritis are rheumatologists. If sacroiliitis is caused by a rheumatic condition, such as ankylosing spondylitis, a rheumatologist will be the appropriate specialist to manage the treatment.
Physical therapists play a necessary role in the management of sacroiliitis by implementing specific exercise programs to strengthen the muscles around the sacroiliac joints, improve mobility and reduce pain.
Physicians specializing in sacroiliitis with Regenerative Medicine therapies.
As a specialist in regenerative medicine, you can offer advanced treatments such as platelet-rich plasma (PRP) injections or cytokine-rich serum therapies to promote healing and reduce inflammation in the sacroiliac joints.
What are the causes of sacroiliitis?
The most common cause of sacroiliac joint osteoarthritis is inadequate mechanical stress on the sacroiliac joints.
- It is usually caused by incorrect or unilateral training or by having legs with different lengths that can change the position of the pelvis.
As a result, the joint cartilage begins to wear down. In turn, the bones involved may change shape and cause misalignment of the pelvis.
SI joint osteoarthritis can occur on the left, right or both sides.
- Other causes are previous injuries in the pelvic area, inflammation of the SI joint or overweight.
- Age-related degenerative wear and tear of the affected joint surface can also lead to the development of SI joint osteoarthritis over time.
What are the symptoms of sacroiliitis?
Those affected often suffer from hip problems, severe low back pain and limited mobility.
Pain occurs suddenly during movement and may radiate to the legs.
For many patients, symptoms are worse in the morning and subside throughout the day.
- Initially, it only occurs when subjected to stress, such as walking or standing for long periods of time.
- As the disease progresses, even small bending and twisting movements of the upper body cause pain.
Affected persons then usually adopt a posture that helps relieve the pain.
If osteoarthritis of the sacroiliac joint becomes chronic, the pain will be permanent and will also worsen with exertion.
From that moment on you will be limited in your daily activities and you will have to live with the pain.
A summary of typical symptoms:
- Hip pain.
- Low back pain.
- Restriction of movement.
Being overweight puts excessive pressure on the SI joint. Therefore, weight loss is an important part of treatment for many sufferers.
Diagnosis of sacroiliac joint pain
Dr. Antonio Vilatela performs the diagnosis of your back pain.
He or she will initially evaluate your medical history and then perform a physical examination or evaluation.
- Through provocative testing, the physician can exert stress on the SI joint and thereby trigger the pain typically associated with SI joint osteoarthritis. This makes it possible to detect the location of the problem.
- Imaging techniques, such as MRI and CT, can aid in diagnosis and give an early indication of whether the cause is aninflammatory process or SI joint osteoarthritis.
- X-rays are used to detect misalignment and wear of the articular surface.
When the condition has progressed, radiographs can provide clear information and show changes in the joint structures.
The images show a reduction of the joint space, as well as wear and tear of the cartilaginous articular areas.
Treatment of sacroiliac joint pain with injections
The first goal of treatment is the reduction of acute symptoms, especially pain in the buttocks and legs.
Most patients with sacroiliac joint osteoarthritis are stabilized with conservative care for 6-8 weeks.
- Use of analgesics and anti-inflammatory drugs or muscle relaxants to relieve pain.
- Physical therapy: exercise programs to help relieve pressure on the nerves that cause your pain. Exercise loosens tight muscles, improves circulation and joint mobility.
- Thermal treatments can relieve pain: hot or cold compresses, or both, applied to the back will help alleviate much of the discomfort and relieve stiffness as well as pain.
When these treatments do not achieve the expected objectives, anti-inflammatory injections, sacroiliac radiofrequency, intra-articular injections of platelet-rich plasma can be considered as preliminary steps that can avoid or delay surgery.
Corticosteroid injections
Spinal injections or infiltrations: Spinal injection, called epidural or nerve block, is an injection of anesthetics and steroids into the spinal column.
They reduce swelling and nerve inflammation, and allow your body to recover and return to activity more quickly.
Treatment with sacroiliac infiltration of platelet-rich plasma
PRP infiltrations are used in traumatology to stimulate the natural healing response that achieves the recovery of mobility, strength and normality of injured tissue.
In these cases, prolotherapy and/or intra-articular PRP injections are a relatively new and increasingly popular option.
Prolotherapy
Prolotherapy is a technique involving the injection of an irritant (hyperosmolar dextrose solution) in the treatment of chronic painful musculoskeletal conditions.
The most frequently published indication is in the treatment of chronic low back pain, but there are recent studies that have examined its use in the treatment of refractory tendinopathies, as well as osteoarthritis.
Prolotherapy may be useful in treating chronic low back pain when combined with complementary therapies, such as spinal manipulation or platelet-rich plasma injections.
The irritating agents used in prolotherapy stimulate stem cell activity and have enormous benefits for nerve and musculoskeletal conditions.
Our regenerative therapy with prolotherapy simultaneously provides pain relief and helps tissue regeneration by activating these stem cells.
Effective application has been documented in patients with sacroiliac joint pain, with benefits that last for many years.
Rather than merely masking symptoms, regenerative medicine treatments aim to cure the underlying pathology that causes pain and disability.
Radiofrequency treatment
Radiofrequency rhizolysis provides longer lasting relief for those suffering from pain due to sacroiliac osteoarthritis compared to cortisone injections and prevents patients from undergoing aggressive treatments such as surgical fusion of the sacroiliac joints.
Spine surgery
If you have severe buttock pain on an ongoing basis and conservative measures do not reduce your symptoms, surgery will be a last resort.
During surgery, a procedure known as arthrodesis may be performed in which the surgeon screws the sacrum and ilium together .

Other causes of back pain
Back pain can have its origin in any of these structures, but in most cases, this pain is not a consequence of any significant damage to the spine.
This pain usually comes from surrounding muscles, ligaments or joints and occasionally from problems in the vertebral discs.
The most frequent pathologies, conditions and treatments for back pain are the following:
- Sacroiliitis joint pain: Process of wear and tear and loss of cartilage in the sacroiliac joint (junction between the support of the pelvis and the lower region of the spine).
- Facet joint pain: Facet joint syndrome is related to the aging process, the loss of cartilage and synovial fluid in the joints of the spine.
- Herniated disc operation: For spinal injury when one of the intervertebral cushioning discs suffers a tear or rupture.
- Pain after back surgery: sometimes patients experience improvement, but as scar tissue forms, leg and back pain reappears or increases.
- Coccyx pain: is pain at the base of the spine at the top of the buttocks. It is one of the most common causes of back pain in sports.
- Spondylolysis: frequent in young athletes, even teenagers, who practice sports such as gymnastics, pole vaulting and soccer.
- Spondylolisthesis: is a condition of the spine that occurs when one vertebra is displaced or has slipped forward or backward over the one below it.
- Muscle back pain or muscle-ligament strain: is the most common sports injury caused by an injury to the soft tissues surrounding the spine.
