What is hip pain?

What is trochanteric bursitis?

The hip joints provide strength and flexibility in walking, running and jumping.

However, hip joints can cause pain from overuse or overexertion.

One of the most common causes of hip pain is inflammation of the lateral part or near the greater trochanter.

Hip bursitis is inflammation of the small fluid-filled sacs called bursae, which provide cushioning between the soft tissues (muscles, tendons and ligaments) and the hip bones.

Most commonly, bursitis causes pain in the hip joint, extending to the outside of the thigh area.

Bursitis on the outer (lateral) side of the hip is called greater trochanteric bursitis or trochanteritis.

It occurs when the bursa on the outside of the hip becomes damaged, irritated or inflamed.

What is Greater Trochanteric Pain Syndrome?

Greater Trochanteric Pain Syndrome (GTPS) is a common condition characterised by pain in the lateral hip region, often associated with tenderness over the greater trochanter, the bony prominence of the femur.

This syndrome typically manifests as chronic pain that may radiate down the outer thigh and into the buttock, and it can be aggravated by activities such as climbing stairs, prolonged sitting, or lying on the affected side.

Symptoms of hip pain

Causes of trochanteric bursitis

This condition is mainly caused by an overuse injury to the outer tendons of the hip.

Certain positions, movements or diseases can cause friction or strain on the greater trochanter bursa and lead to trochanteritis. When the bursa is injured, the muscles and tendons do not glide smoothly, which can cause pain.

Other causes of trochanteritis:

  • Muscle tears: a hip strain, which occurs when a muscle tears or pulls beyond its limit around the hip joint, can lead to inflammation and irritation of the bursae of the greater trochanter.
  • Direct trauma (blows or falls on the side of the hip), a muscle or tendon tear.
  • Activities that overload or injure the joint areas such as running up stairs, prolonged standing, cycling…, can put excessive stress on the joints. This can lead to hip injury and trochanteric bursitis.
  • Complications of hip surgery: previous surgeries or complications with surgeries can cause bursitis by irritating the bursa.
  • Poor posture: Incorrect posture leading to scoliosis and other conditions can lead to this type of hip pain.
  • Certain diseases or conditions: gout, rheumatoid arthritis, thyroid diseases, spinal diseases, and even unusual reactions to medications or infections can cause trochanteric bursitis.
  • Calcium deposits: This can irritate the bursa and cause inflammation if this calcium deposit is located within the tendons connected to the trochanter muscles.
  • Weakness of the hip muscles is an essential factor. Buttock pain is twice as likely in women, with a higher rate in older women.

An irritated and inflamed bursa causes pain that can limit range of motion and make some activities or positions uncomfortable.

Symptoms of trochanteric bursitis

Pain on the outside of the hip is usually the first thing you notice.

The pain is worsened by lying directly on the hip, going up and down stairs, and prolonged walking and running.

It may also hurt when crossing the legs. In addition, the outside of the hip is tender to the touch, and pain occurs when moving the leg outward.

Occasionally, people with buttock pain may have significant night pain.

 

Diagnosis of trochanteric bursitis

Dr. Miguel Porcar will review your symptoms, medical history and perform a physical examination, focusing on how long you have had the condition and the specific movements that are causing your pain.

In addition, a general examination of the hip and spine excludes other causes of pain outside the hip such as hip arthritis, referred sacroiliac joint pain or lumbar spine.

Other tests for diagnosis may include:

  • X-rays: do not show the soft tissues such as the bursae of the hip and are therefore usually normal. However, they are requested to rule out other problems.
  • MRI and ultrasound: create better images of the soft tissues and may show fluid or inflammation in the bursa.
  • Ultrasonography shows trochanteric tendinitis and bursitis. Sometimes, calcifications or tendon tears can be seen. Simply having trochanteric bursitis is often associated with gluteal tendinitis.
  • A simple way to diagnose trochanteric bursitis is by injection of local anesthetic directly into the bursa. If the injection eliminates the pain immediately, the diagnosis is probably trochanteric bursitis.

Treatments for trochanteric bursitis: hip injections and surgery

Although trochanteric bursitis can be painful and uncomfortable, there are many effective treatments for those experiencing this hip pain.

Initially we start with rest measures, ice, analgesics, rest and activity modification, avoiding activities or sports that cause impact on the hip.

  • Modifying activities to relieve pressure on the hip can significantly aid healing.
  • Applying ice to the inflamed area reduces swelling and inflammation, helps relieve pain and tenderness caused by bursitis.
  • Injections of non-steroidal anti-inflammatory drugs (NSAIDs).
  • Physical therapy exercises for strengthening and stretching, avoiding activities that cause pain. This helps prevent muscle atrophy and can also be used to prevent trochanteritis, not just treat it.

If these treatments do not resolve the bursitis, fluid may need to be removed from the bursa. Or you may need steroid injections to reduce pain and swelling.

Is a steroid injection good for trochanteric bursitis?

Yes, a steroid injection is effective for trochanteric bursitis. It quickly reduces inflammation and relieves pain in the outer hip area. Relief may last weeks to months. It’s a common treatment when rest, ice, or medication are not enough.

For longer-term healing, platelet-rich plasma (PRP) injections are also beneficial, especially in chronic cases. Ultrasound-guided injections with platelet-rich plasma are very effective and delay or even avoid surgery.

Is PRP better than cortisone for greater trochanteric bursitis?

PRP may offer longer-term relief than cortisone by healing tissue, not just reducing inflammation. Cortisone works faster but relief is often temporary. PRP takes longer to act but can improve function and reduce pain for many months. The best option depends on each case.

Corticosteroid injection

Using ultrasound-guided techniques, we can administer an injection of local anesthetic and a cortisone preparation.

Cortisone is a very effective anti-inflammatory drug, and injecting it into the bursa relieves pain and reduces inflammation.

You may need more than one injection, usually within a few months of each other.

However, this option should be limited to only a few times to avoid damaging the surrounding bursa tissue.

 

Treatment of trochanteric bursitis or Greater Trochanteric Pain Syndrome with Platelet Rich Plasma Injectión on Hip

We use the treatment of trochanteritis with Platelet Rich Plasma injections.

It is a very effective procedure that is an alternative prior to surgery.

Patients with greater trochanteric pain syndrome, especially in association with gluteus medius minimus or fascia lata tendinopathies, have better and longer lasting clinical outcomes when treated with ultrasound-guided PRP injections compared to those treated with cortisone.

We administer a concentrated dose of platelets prepared from your own blood to repair damaged tendons.

Patients experience improvement in their symptoms, with a significant decrease in pain and an increase in functionality within one to two weeks of receiving the injections.

Trochanteric Bursectomy

If the more conservative methods and injections do not yield the desired results, we can consider surgical treatment.

The main objective is to remove the thickened bursa, eliminate bone spurs that may have formed in the greater trochanter, and relax the large gluteus maximus tendon .

Some surgeons prefer to lengthen the tendon slightly, and others prefer to remove a section of the tendon that directly abuts the greater trochanter.

If trochanteric bursectomy is necessary, physical therapy sessions may be required for up to two months after surgery.

The initial sessions of physical therapy will focus on controlling pain and swelling following the procedure.

Next, you will begin performing exercises to gradually stretch and strengthen the muscles surrounding the hip joint.

The therapist will help you retrain these muscles to ensure that the head of the femur moves smoothly in the socket and will provide tips on how to perform your activities without straining the hip joint.

Causes of Hip Pain Treatable by a Hip Specialist
