What is plantar fasciitis?
Plantar fasciitis is a painful inflammation of the plantar fascia, which is a thick band of tissue on the bottom of the foot, extending from the heel to the toes.
The plantar fascia can become thickened and inflamed due to irritation at its junction at the base of the heel bone (calcaneus), causing pain in the arch of the foot and heel. In some patients, small tears may occur and these people suffer acute pain that can be unbearable .
Sometimes heel spurs (bone spurs) can be seen on x-rays, but these are not usually the cause of the patient’s heel pain.
It should be noted that “heel spur” is a misdiagnosis that has been used by podiatrists and physicians for many years. Today, more and more professionals are open to the fact that heel spurs are not the problem and that these bone spurs do not cause pain.
Instead, the source of pain is inflammation within the plantar fascia surrounding the spur itself.
Fasciitis occurs most often in adult men and women, although it can also occur in people who are constantly on their feet.
Causes of plantar fasciitis
The plantar fascia functions as a shock absorber and supports the arch of the foot.
- Excessive pressure on the fascia can strain and tear the tissue, causing heel pain.
- Over-stretching or repeated overuse causes irritation or inflammation of the fascia.
Other risk factors or causes that favor the appearance of plantar fasciitis are:
- Very overweight or obese people .
- Problems with the arch or curvature of the foot such as flat feet.
- Tight calf muscles and wearing soft shoes are a problem.
- Frequent long-distance running, ballet and aerobic dance.
- Jobs that require walking or standing on hard surfaces for long periods of time.
- Use of thin-soled shoes or shoes with little support for the arch of the foot.
- Biomechanical problems that cause foot malfunction can also trigger the disease.
- Patients who suddenly increase the amount or frequency of sports/exercise activity are also at risk.
There is no single cause of plantar fasciitis, it is multifactorial, as is the treatment of plantar fasciitis.
What are the symptoms of plantar fasciitis?
Signs and symptoms include:
Throbbing pain in the sole of the foot and heel.
- Heel pain is common first thing in the morning when you get out of bed.
- It reappears when standing up again after periods of rest, such as when driving or sitting.
- Heel pain can be erratic and there are often pain-free periods when you may think it is healing.
- If the pain comes on very suddenly, rather than gradually, it may be due to a tear of the plantar fascia.
Diagnosis of plantar fasciitis
Dr. Miguel Alfonso Porcar will initially evaluate your medical history and then perform a physical examination or evaluation.
He or she will examine your foot for signs of flat or cavus foot, tenderness, swelling, redness, stiffness, or tightness of the arch of the foot.
More advanced imaging is often helpful in accurately defining the origin of the pain and therefore the best treatment for each individual.
- It is important to differentiate between normal plantar fasciitis and plantar fascia tear, as the treatment is different.
- An x-ray may be necessary to rule out a stress fracture, which sometimes causes similar symptoms.
- Ultrasound helps us to accurately diagnose the cause of the patient’s heel pain.
An x-ray, ultrasound or MRI may be necessary to rule out other causes of heel pain, such as a stress fracture or pinched nerve.
Treatments for plantar fasciitis and heel pain
Our goal is to provide a long-term solution to your heel pain, not just quick pain relief.
We are specialists with extensive experience in the treatment of plantar fasciitis, as well as all other types of painful heel conditions.
Initial treatments usually include rest from activities that make the pain worse. Application of ice packs to the area at least 2 times a day and use of night splints that stretch the plantar fascia while you sleep.
Other treatments for heel pain caused by plantar fasciitis include:
- Orthotics offload and support the plantar fascia, allowing the foot to heal naturally.
- Shoe changes/recommendations: use shoes with a good support or heel cup and good cushioning capacity.
- Custom-made prescription orthoses (soft arches).
- Treatment with non-steroidal anti-inflammatory drugs(NSAIDs).
- Inject corticosteroids directly into the plantar fascia to reduce pain and inflammation.
- Physiotherapy: our physiotherapist will design an exercise program focused on stretching the plantar fascia and Achilles tendon, and strengthening the muscles of the lower leg.
- Shock wave treatment: sound waves are directed to the painful area to stimulate the healing process.
Plantar fasciitis treatment with PRP Platelet-Rich Plasma
Multidisciplinary pain management in more complex cases with pain, we can perform platelet-rich plasma injections (abbreviated as PRP) as an option prior to fasciectomy surgery.
Platelet-rich plasma therapy is a regenerative treatment for plantar fasciitis that involves injecting your own platelets to activate the body’s natural repair and regeneration processes.
