What is elbow pain?

What is golfer’s elbow?

The elbow is a joint formed by three bones: the humerus, ulna and radius.

The lower end of the humerus has bony protrusions called epicondyles, which serve as insertion sites for the major tendons and muscles that aid in arm movement. Specifically, the protrusion on the inside of the elbow is called the medial epicondyle.

Golfer’s elbow, also called epitrochleitis or condition of the medial epicondyle region, is a painful pathology caused byrepeated muscle contractions in the forearm and neighboring joints.

These muscle contractions cause tendon contraction, which, if repeated, can lead to inflammation and injury to the tendons that attach to the medial epicondyle. Epitrochleitis is therefore a process of tendinitis, i.e. inflammation of the tendons.

Chronic epitrochleitis

During the initial stages of epitrochleitis, an inflammatory reaction is triggered in the tendon which, over time, may become persistent and evolve into a chronic form of the disease.

This chronic form of epitrochleitis is characterized by prolonged duration, with degeneration of tendon fibers and small tears in the tendons of the pronator teres and flexor carpi radialis muscles, but without signs of inflammation.

Inner elbow pain, commonly known as “golfer’s elbow” or epitrochleitis, is a condition that affects the inner part of the elbow. It is characterized by pain and tenderness, especially over the bony prominence on the inner side of the elbow, where the tendons of the forearm muscles attach to the bone.

This type of pain is usually the result of overloading or overuse of the muscles and tendons of the forearm, which may become inflamed or suffer small tears.

This condition is similar to tennis elbow, but affects the inside of the elbow instead of the outside. Although it is called golfer’s elbow, it can affect anyone who performs repetitive activities that involve gripping, twisting or lifting.

Causes of epitrochleitis or golfer’s elbow

Golfer’s elbow is usually caused by overuse of the forearm muscles and tendons that control the movement of the wrist and fingers.

It occurs especially when poor technique or improper equipment is used when striking the ball.

Other common causes are any activity that requires repetitive motion of the forearm, such as painting, hammering, typing, or throwing sports.

At other times it may be caused by direct trauma, such as a fall, car accident or work-related injury.

What are the symptoms of golfer’s elbow?

The main symptoms of epitrochleitis are:

  • Elbow pain that comes on suddenly or gradually.
  • Pain in the inner aspect of the elbow during activity.
  • Elbow stiffness with decreased range of motion.
  • Weakness of grip and increased pain when grasping objects.
  • Increased pain in the elbow when the wrist is flexed, the elbow is extended or the palm of the hand is placed upwards.

Diagnosis of Golfer’s elbow

Dr. Miguel Alfonso Porcar will evaluate tennis elbow by reviewing your medical history and performing a complete physical examination.

We will also perform imaging tests such as X-rays, ultrasound, magnetic resonance imaging and/or electromyogram (EMG) to rule out any nerve compression.

  • A thorough clinical examination, often accompanied by an X-ray, is necessary to rule out a fracture or localized arthritis as the cause of your pain.
  • More advanced imaging is useful to accurately define the origin and the best treatment.
  • At the Regenerat clinic we use high resolution ultrasound, useful to identify factors that can guide the best treatment. It can also help to exclude other isolated or coexisting causes of pain.

Treatments for golfer’s elbow

There are a variety of treatments available for people experiencing medial epicondylitis or epitrochleitis.

Once the diagnosis and severity of golfer’s elbow is confirmed, Dr. Alfonso Porcar can recommend several treatment options.

  • Activity restrictions: starting with modifying or limiting use, and resting the arm from activities that worsen symptoms.
  • Rest and ice applied to the injury will help reduce swelling and pain.
  • We may also recommend the use of orthoses such as splints or braces to reduce the stress on the injured tissues.
  • Analgesics and anti-inflammatory drugs and/or steroid injections may also be prescribed to treat pain and swelling.
  • Physiotherapy exercises to strengthen the injured muscles and tendons, once the initial symptoms have subsided.
  • Injections of prolotherapy or infiltrations of platelet-rich plasma.

Platelet Rich Plasma Injections Treatment of the Elbow

Multidisciplinary pain management for complex cases of chronic epicondylitis may include platelet-rich plasma (PRP) injections and other forms of regenerative medicine as options prior to surgery.

  • Use a concentrated dose of platelets prepared from your own blood.
  • Prior to the injection, the patient undergoes a blood draw. After processing, the PRP is obtained, which has proven to have a reparative and regenerative capacity known as Platelet Rich Plasma.
  • Once the PRP is obtained, the infiltration is performed with the aid of ultrasound.
  • PRP accelerates recovery and healing of tendon, joint and muscle injuries.

Surgery for golfer’s elbow

If both conservative treatment and PRP infiltrations do not resolve the condition and symptoms persist for 6 to 12 months, Dr. Miguel Alfonso Porcar may recommend surgery to treat golfer’s elbow.

The goal of surgery is to remove diseased tissue inside the elbow, improve blood supply to the area to promote healing and relieve symptoms.

There are several options for medial epicondyle release surgery.

  • Dr. Miguel Alfonso Porcar performs this operation on an outpatient basis, so you can start recovering at home the same day you undergo the procedure.
  • We make a small incision on the side of the elbow and section the tendon to remove the damaged and inflamed part.
  • We remove any scar tissue or other debris that may cause inflammation, and secure the healthy tendon in place using stitches.
  • After closure of the incision with stitches, we apply a bandage and place a soft immobilization on the arm. This gives the tendon a chance to heal.

After surgery, physical therapy treatments are prescribed to improve range of motion and joint strength.

Hace tres meses acudí a la clínica Regenerat de Torrens, desesperada por un desgarro en el hombro , que me causaba mucho dolor . Agradecidísima es poco. Le debo todo. Soy profesional del running y tuve que dejar de correr durante una temporada. Me diagnosticaron una condropatía rotuliana que nadie supo darme una solución. Causes of elbow pain

Causes related to elbow injuries include damage to parts of the elbow, such as bones, cartilage, ligaments, tendons or muscles.

 

  • Elbow osteoarthritis: occurs when the cartilage surface of the elbow is worn or damaged by previous injury, such as a dislocation or elbow fracture, or by degeneration.
  • Lateral epicondylitis: refers to pain on the outer side of the elbow. It is the result of repetitive strain that causes an injury to the insertion of the tendons at the epicondyle of the elbow.
  • Golfer’s elbow – Medial epicondylitis: causes pain in the internal face of the elbow (the medial epicondyle), which can radiate towards the internal face of the forearm. Its pathogenesis is similar to the previous one.
  • Olecranon bursitis treatment: Bursitis is the inflammation of the elbow bursa (a fluid-filled sac-like structure that reduces friction between bones, muscles and tendons). Direct trauma, a localized infection or overuse can be the cause, among others.

The most effective treatment for many of these ailments is often the infiltration of the elbow with Platelet-Rich Plasma injections.
