What is Achilles tendinitis: Inflammation of the Achilles tendon?

Achilles tendonitis occurs when there is irritation and inflammation in the Achilles tendon, a large tendon that attaches the calf muscles to the heel bone, or calcaneus.

The Achilles tendon is used for jumping, walking, running and support on the sole of the foot.

Overuse or damage to this area can lead to Achilles tendonitis.

There are two types of Achilles tendinitis:

  • Non-insertional Achilles tendinitis involves small tears in the fibers of the middle portion of the tendon and tends to affect young adults who are active.
  • Insertional tendonitis affects the lower part of the tendon where it attaches to the heel bone. It can affect people of any age, even those who are not physically active.

Conservative treatments can help relieve pain and heal Achilles tendonitis.

However, if conservative treatment does not work, or if the Achilles tendonitis worsens, the tendon may rupture.

Symptoms of foot pain and ankle pain

Causes of Achilles tendinitis – inflammation of the Achilles tendon

Achilles tendonitis is usually caused by overuse of the tendon during exercise or gradual wear and tear as you get older.

It can also be caused by arthritis, especially among middle-aged and older adults.

Other common causes of Achilles tendinitis are:

  • Exercising without warming up.
  • Straining the calf muscles during repeated movements.
  • Practicing sports, such as tennis, paddle tennis, badminton… that require quick stops and changes of direction.
  • Running long distances, too intensely or uphill too often.
  • Wearing old footwear or footwear that does not support your foot properly.

What are the symptoms of plantar fasciitis?

The main symptoms of Achilles tendon pain are:

  • Pain and swelling in the back of the heel when walking or running and to the touch. This pain may also appear the day after exercise.
  • You may also have tight calf muscles.
  • Limited range of motion when flexing the foot.
  • Stiffness and pain in the Achilles tendon when you get out of bed in the morning.
  • Heat around the heel or along the tendon.
  • Difficulty standing on tiptoe.

Diagnosis of Achilles tendonitis

Dr. Miguel Alfonso Porcar will initially evaluate your medical history and then perform a physical examination of the calf, heel, foot and ankle to look for symptoms associated with Achilles tendonitis.

He will also palpate the affected area to elucidate the exact location of the pain or discomfort.

Subsequently, we can perform imaging tests to confirm the diagnosis of Achilles tendinitis.

These tests include:

  • X-ray to see the bones of the leg and foot to check for bone calcification.
  • Magnetic resonance that allows us to detect tendon ruptures or tissue degeneration.
  • Ultrasound to check if the tendon is damaged, thickened or inflamed.

Treatments for Achilles tendinitis

There are many treatments for Achilles tendonitis; from home remedies such as rest and anti-inflammatories, to steroid injections, platelet-rich plasma (PRP) injections and surgery.

Home treatments for Achilles tendonitis include reducing physical activity and switching to low-impact exercises such as swimming, applying ice, stretching to strengthen the calf muscles, wearing boots or shoes with a reinforced heel to prevent heel movement.

    Other non-surgical treatments for the treatment of Achilles tendon pain include:

    • Orthopedic insoles that allow the foot to heal naturally.
    • Shoe changes/recommendations: use shoes with a good support or heel cup and good cushioning capacity.
    • Custom-made prescription orthoses.
    • Treatment with non-steroidal anti-inflammatory drugs (NSAIDs).
    • Physiotherapy: our physiotherapist will design an exercise program focused on stretching the plantar fascia and Achilles tendon, and strengthening the muscles of the lower leg.
    • In addition to exercises, the application of sports tape to support the bottom of the foot may also help alleviate symptoms.
    • Shock wave treatment: sound waves are directed to the painful area to stimulate the healing process.

    Platelet-Rich Plasma Injections for Achilles tendonitis

    Platelet-rich plasma therapy (abbreviated as PRP) is a regenerative treatment for Achilles tendon pain or Achilles tendonitis.

    It consists of the infiltration of platelets extracted from your own blood, to activate the body’s natural repair and regeneration processes.

    This biological therapy induces the mechanisms for healing, and is the treatment of choice to eliminate the pain and inflammation of the Achilles tendon produced by tendinitis, prior to surgery.

    Achilles tendon surgery

    If non-surgical treatments do not provide the desired results the Achilles tendon.

    If the condition worsens and is left untreated, there is an increased risk of rupture of the Achilles tendon, which requires emergency medical attention. This can cause severe pain in the heel area.

    Surgery may consist of

    • Lengthen the calf muscles (calves).
    • Remove bone spurs (extra bone growths), damaged tendon tissue, or both.
    • Tendon repair.

    Causes of Foot pain – Ankle pain

    Anyone can be at risk for foot or ankle pain, as it can result from injury or everyday wear and tear.

    Some factors can increase the likelihood of a person suffering from foot and ankle pain include the following: 

    • Osteoarthritis in ankle treatment: Occurs when the surface of the cartilage of the ankle begins to wear away. This causes inflammation, stiffness and pain in the joint.
