What is patellar tendinitis?
There are 3 different types of tendinitis:
Knee tendinitis – quadricipital tendinitis and goosefoot tendinitis.
Runner’s or jumper’s knee, also known as “patellar tendonitis,” is an inflammation of the patellar tendon that connects the kneecap to the shinbone.
The patellar tendon plays a fundamental role in the use of the leg muscles.
This tendon assists in the extension of the lower leg.
It helps extend the leg muscles to perform movements such as kicking a ball, running uphill and jumping in the air .
Causes of knee tendinitis
Patellar tendinitis occurs when the tendon that connects the patella to the tibia becomes irritated from overuse, which causes repetitive strain on the patellar tendon.
This continued stress causes small tears that irritate the tendon. As the tears in the tendon multiply, they cause pain and weakness in the tendon.
If you ignore the pain and continue with your activity, you can lead to further tears, increased pain and reduced functionality and mobility of the knee joint, which can lead to severe patellar tendinopathy.
It is often seen in athletes who perform repetitive jumping, so patellar tendinitis is often referred to as “jumper’s knee”.
Patellar tendinitis is often seen in track and field, basketball and volleyball, although it can also be seen in runners and other types of athletes.
It accounts for about 5% of all running injuries.
Rarely, this condition can also occur due to an acute tendon injury that has not healed properly.
What are the symptoms of knee tendinitis?
Knee pain is a common sign of patellar tendinitis.
There may be pain and swelling between the patella and the place where the tendon attaches to the tibia.
At first, knee pain may appear only when you start exercising or right after an intense workout.
This injury may worsen over time until it begins to interfere with your daily activities.
Finally, it will influence daily movements, such as climbing stairs or getting up from a chair.
How is an inflamed knee tendon diagnosed?
Dr. Alfonso Porcar expert in the treatment of tendinitis of the knee of the runner and jumper performs the diagnosis of knee pain.
- He will initially evaluate your condition with your medical history, your symptoms and a physical examination or evaluation.
- We may perform a high-resolution ultrasound-guided ultrasound to evaluate the injured tissues in real time to ensure that there are no problems affecting the bones or the bone spur around the knee.
- In cases where it may become necessary, we may order additional imaging tests.an MRI may reveal degenerative changes in the patellar tendon.
Treatments for patellar tendinitis
More conservative options include resting the injured knee and avoiding activities such as running and jumping.
Stretching before exercise is important to prevent recurrence of patellar tendonitis. These exercises can also help strengthen the patellar tendon and nearby muscles, such as the quadriceps, hamstrings and calf muscles.
Applying ice to the affected area helps to control inflammation and reduce swelling.
A knee brace or strap (called a patellar strap or Chopat strap) can be used to relieve pain by directing force away from the tendon and toward the strap.
Infiltration treatments
Nonsteroidal anti-inflammatory drugs may be prescribed to relieve the pain and swelling associated with patellar tendinitis.
Corticosteroid injection
With ultrasound-guided techniques, an injection of corticosteroids can be administered directly into the sheath surrounding the affected patellar tendon.
The use of ultrasound is essential to avoid introducing the corticosteroid intratendon and to perform it in the sheath. This helps to relieve pain and perform strength exercises without any pain.
Prolotherapy
It is a technique that involves the injection of an irritant, usually a hypertonic dextrose solution (dextrose concentration above 12.5%).
Prolotherapy is commonly applied in the management of chronic joint pain such as knee, low back and shoulder.
It is recognized as a reliable and effective treatment method for certain conditions, including osteoarthritis, tendonitis, bursitis, as well as muscle and ligament strains.
Several studies have shown the ability to tighten and strengthen lax anterior cruciate ligaments, improving knee osteoarthritis.
Our regenerative therapy with prolotherapy simultaneously provides pain relief and helps tissue regeneration by activating these stem cells.
Knee tendinosis treatment with PRP injections
- PRP is a blood-derived product, which does not contain stem cells.
- Accelerates recovery and healing of tendon, joint and muscle injuries.
- It is used as an adjuvant in surgeries to improve the result.
- Cures sports injuries.
Treatment with injections in the knee with platelet-rich plasma or PRP is effective in treating patellar, quadricipital or goosefoot tendinitis.
It helps accelerate healing and reduce inflammation of the injured tendon. In addition, PRP injection is safe and relatively painless.
Do PRP injections work for patellar tendonitis?
Platelet-rich plasma (PRP) injections are a promising treatment for patellar tendinopathy (jumper’s knee). A systematic review in the American Journal of Sports Medicine found significant improvements in pain and function compared to corticosteroids or placebo.
You can read below the opinions of patients from our clinic who have already had their tendinitis treated with PRP injections.
Orthopedic surgery
In rare cases, such as when pain persists after the application of bioregenerative therapies, surgery may be considered.
Surgery involves removal of the severely damaged parts of the tendon and repair of any tendon tears, associated with regenerative therapies such as platelet-rich plasma mesh around the repaired area.

- Osteoarthritis stages treatment: osteoarthritis occurs when joint cartilage wears away and causes pain, stiffness and swelling.
- Chondromalacia patellae: is the softening and loss of cartilage in the kneecap. It occurs in young and adult athletes suffering from knee arthritis.
- PRP injections for knee bone marrow edema: Bone marrow edema is inflammation or injury in the medullary zone of the knee, often caused by a contusion, leading to microfractures and swelling.
