What is shoulder bursitis?

The shoulder is made up of three bones: the humerus, the scapula and the clavicle.

The arm is held in the shoulder joint by the rotator cuff.

These muscles and tendons form a sheath around the head of the humerus.

Between the rotator cuff and the bone at the top of the shoulder (acromion) is a lubricating sac called a bursa. The bursa allows the rotator cuff tendons to slide freely when you move your arm.

Inflammation of the bursa is known as shoulder bursitis or rotator cuff bursitis.

Causes of shoulder bursitis or rotator cuff bursitis

Subacromial (below the acromion) or rotator cuff bursitis is usually caused by minor or inadvertent injuries to the shoulder joint associated with certain professions or sports activities.

Rotator cuff pain is common in both young and middle-aged athletes .

  • Young athletes who use their arms above the head for: swimming, baseball, tennis… among others, are especially vulnerable.
  • People who repeatedly lift objects or perform overhead activities, such as hanging paper, building or painting, are also susceptible.

Other causes are:

  • Bony growths of the acromion process.
  • Natural degeneration of tendons due to age, especially in people over 40-50 years old.
  • They can also be caused by high-impact trauma during a fall or a traffic accident.

Symptoms of shoulder bursitis

Rotator cuff pain usually causes swelling and local tenderness in the front and side of the shoulder.

There may be pain accompanied by functional impotence in any movement of the shoulder, especially in abduction(separating the shoulder from the body to the side) and rotations(putting the hand behind and in front of the body).

Initial symptoms may be mild, and therefore patients often do not seek treatment at an early stage (although this is also recommended).

These symptoms may include pain, tenderness, increased volume and redness.

  • Mild pain that is present both with activity and at rest.
  • Pain radiating from the front of the shoulder down the side of the arm.
  • Sudden pain when lifting and reaching for objects.

As the problem progresses, the symptoms increase:

  • Pain at night. Inability to rest at night.
  • Loss of strength and range of motion.
  • Difficulty performing activities in which the arm is placed behind the back, such as buttoning or closing zippers.

Diagnosis of shoulder bursitis

Dr. Miguel Alfonso Porcar will review your symptoms, medical history and perform a physical evaluation to check range of motion, stability and joint strength.

It will check for tenderness or pain in any area or if there is any obvious deformity.

To measure the range of motion of the shoulder, I will perform a thorough examination of the joint as well as neighboring regions that may involve the shoulder.

Other tests that can help us confirm the diagnosis are:

  • X-rays: X-rays do not show the soft tissues of the shoulder such as the rotator cuff and are therefore usually normal. However, they are useful to rule out other types of injuries prior to effective treatment.
  • Ultrasound: creates better images of the soft tissues and may show fluid or swelling in the bursa, and/or a partial rotator cuff tear.
  • Echo-guided infiltration: we can inject a drug into the space below the acromion to relieve pain. If the pain is relieved, it indicates that the problem was tissue impingement due to bursitis or tendonitis.
  • MRI: as a last resort, it will be the test of choice for both bursa and rotator cuff tendon injuries. It performs a broader and more detailed study of the region without irradiating the patient.

Shoulder bursitis: Treatment without surgery

Treatment of rotator cuff bursitis begins with rest, ice, analgesics and/or anti-inflammatory measures.

  • Rest and activity modification, avoiding activities with overhead arm movements.
  • Anti-inflammatory and/or analgesic medications to alleviate the symptoms.
  • Immobilizing orthoses or functional bandages to increase joint rest.
  • Physical therapy to restore normal shoulder motion. Specific stretches of the posterior capsule can be very effective in relieving shoulder pain.

 

Corticosteroid injection

With ultrasound-guided infiltration, we can administer an injection of local anesthetic and a corticosteroid preparation at the most clinical point of the lesion; never intra-tendinous.

Steroid injections into shoulder are a safe medication in most patients. This drug provides temporary pain relief and allows for increased joint function.

At the Regenerat clinic, we have state-of-the-art ultrasound equipment, by means of which a detailed and individual exploration of each patient is performed prior to the infiltration.

Once the lesion has been studied, the injection is applied in the exact or closest point , with the objective of providing the patient with the greatest benefit.

Platelet Rich Plasma Infiltration for Shoulder Bursitis

The infiltration with platelet-rich plasma for the treatment of shoulder bursitis is an effective pre-surgical procedure that uses a concentrated dose of platelets prepared from your own blood.

Prior to the injection, the patient undergoes a blood extraction. After a period of processing, a substance is obtained that has proven to have reparative and regenerative capacity known as Platelet Rich Plasma.

It is a safe technique, since the substance is derived from the patient himself, so it is very difficult for there to be adverse effects after the infiltration. The clinic ensures that the processing of the sample is optimal and then the infiltration is performed with the help of ultrasound.

As PRP injections are performed at the Regenerat clinic, no hospital stay or surgical recovery period is necessary.

It presents neither the risks nor the costs of surgery.

The results are promising. It implies a more effective benefit in the long term and with the security of being a substance derived individually from each patient.

Shoulder bursitis: Operation by arthroscopy

Surgery for shoulder bursitis is indicated when the patient does not experience improvement after the use of non-surgical treatments.

Surgical intervention can be performed by open or arthroscopic surgery.

The surgical procedure consists of the removal of any bony overgrowth at the anterior end of the acromion bone, as well as any inflammation of the bursa.

In this way we create more space for the rotator cuff and allow unrestricted movement of the joint.

Through two or three tiny incisions in the shoulder, a camera is introduced inside the shoulder to explore the injured joint. Through the remaining incisions, the procedure is performed, thus avoiding a large scar and further patient recovery.

After surgery we will place your arm in a sling for a short period of time for a faster recovery. And as soon as possible we will remove the sling to start exercising and strengthening the muscles.

We will then recommend a rehabilitation program based on your needs and the results of the surgery. This will include exercises to restore shoulder range of motion and arm strength.

  • Dislocated shoulder: Shoulder dislocation is a serious injury involving a momentary loss of contact between the bones of the shoulder joint.
  • Calcifications in the shoulder: appearance of small calcium deposits, usually in the supraspinatus tendon.
