What is osseous edema of the knee?
Bone edema in the knee is an internal bone involvement that can cause swelling and pain as a result of a strong impact or repeated small impacts. This type of injury directly affects the bone tissue, causing swelling in the bone and accumulation of fluid in the bone that causes swelling in the bone marrow.
Knee edema is a common ailment that can affect people of all ages.
Bone edema can also occur in other joints and is called bone marrow edema in the bones of the foot, ankle, hip.
Causes of osseous edema of the knee
- It is usually related to osteoarthritis, often referred to as “wear and tear” arthritis, in which the cartilage breaks down and the joints become painful.
- It may be due to a fracture, bone contusion or trauma, or a joint injury. They are the main cause of bone edema of the knee.
- A bone infection (osteomyelitis) can sometimes cause an accumulation of fluid in the knee.
Osteoarthritis
If you have osteoarthritis, bone edema may be a sign that your disease has worsened and you may have developed subchondral cysts.
The cartilage hardens and forms fluid-filled sacs (cysts) in the joint that wear down the cartilage further, until the bone rubs against the bone. You may feel a lot of pain and have less mobility.
Bone edema due to injury
Spinal bone edema can occur with fractures and other serious bone or joint injuries, which can accumulate different types of fluid in a bone.
This may be blood or fluids released by fibrosis (scar tissue) or necrosis (tissue death).
Bone injuries caused by a direct blow, shearing forces, impact of several bones against each other, may result in a bone hematoma.
Knee bone edema can be acute, following a recent injury, or chronic, due to recurrent microtrauma.
Other causes of intraosseous edema
- Stress fractures of the foot, hip, ankle or knee due to repeated impact and stress.
- Rupture of the anterior cruciate ligament (ACL), which can cause bruising and inflammation of the connective tissue, called synovitis.
- Bone tumors, in which the fluid weakens the bone and increases the risk of fracture.
- Bone infection (osteomyelitis)
Diagnosis of osseous edema of the knee

Treatment of bone marrow edema of the knee with injections
Some types of medullary bone edema of the knee are more difficult to treat than others.
If the problem is related to trauma or repetitive motion, it is usually cured with conservative treatment. This means rest, unloading or partial loading of the limb with crutches is important in the first few days, non-steroidal analgesics and physiotherapy treatment.
Severe cases may require steroid injections, platelet-rich plasma or surgery.

How is edema treatment with PRP and intraosseous infiltrations performed?
The technique of intraosseous infiltrations in the knee consists of performing a subchondral infiltration in the knee on the subchondral surfaces of the tibial plateau and femoral condyle.
