What is wrist pain

What is hand and/or wrist pain?

Wrist pain is defined as any pain or discomfort in the wrist.

The wrist consists of two forearm bones, the radius and ulna, and eight small carpal bones in the palm of the hand.

Bones join together to form multiple large and small joints. Trauma or injury to one or more of these bones causes pain in the wrist.

The wrist is a joint in the body that is often injured.

Wrist pain can occur due to sprains and strains, arthritis and fractures that can occur from lifting and carrying heavy objects, operating machinery, forcing against a fall or from sports-related injuries.

The orthopedic surgeon Dr. Miguel Alfonso Porcar will be able to diagnose the precise causes of your symptoms, and offer you the most appropriate treatment to solve your hand or wrist pain and prevent it from recurring.

Mild cases of pain can be corrected with self-care and rest, but in some more severe cases, physiotherapy treatments , regenerative medicine and orthopedic surgery may be necessary.

Causes of wrist pain or pain in the hand

Hand or wrist pain causes

Some of the most common causes of hand and wrist pain are sprains and strains.

A sprain refers to an injury to a ligament, while a strain refers to a muscle injury. Sprains and strains occur due to excessive force applied during a stretching, twisting or pushing action.

Other causes of pain in the hands and/or wrists include:

  • Hand osteoarthritis: A degenerative joint disease that is the most common type of arthritis affecting older people. It causes the cartilage, the tissue that cushions and protects the ends of the bones in a joint, to wear away, resulting in painful bone-on-bone contact.
  • Thumb arthritis: Arthritis affecting the base of the thumb, i.e. the progressive destruction of the articular cartilage between the trapezium and the first metacarpal.
  • De Quervain’s tenosynovitis: Also called radial tenosynovitis, this is a painful condition affecting the tendons of the first extensor compartment, producing pain at the level of the anatomical snuffbox.
  • Trigger finger: A condition in which the finger locks or catches if you try to straighten or bend it.
  • Carpal tunnel syndrome: A condition characterized by numbness or pain in the thumb and first three fingers. It occurs when the median nerve is compressed at the wrist. It is usually a common complaint in people who use their hands for prolonged periodsin particular occupations, such as computer work.
Symptoms of wrist pain and/or hand pain

What are the symptoms of hand nad/or wrist pain?

Depending on the cause of wrist pain, it may be accompanied by the following symptoms:

  • Swollen fingers.
  • Difficulty in making a fist or grasping objects.
  • Numbness or tingling sensation in the hands that worsens at night.
  • Pain, numbness or tingling, sudden sharp pain in the hand.
  • Swelling or redness around the wrist.
  • Heat in the wrist joint.
  • Restriction of hand and wrist movement.

Call your doctor immediately if your wrist is hot and red and if you have a temperature above 38°C.

These symptoms could indicate infectious arthritis, which is a serious disease.

You should also contact your doctor if you cannot move your wrist or if your hand looks abnormal. You may have broken a bone.

Risk Factors

  • Repetitive movements: when the body performs certain repetitive movements, the tissue can become inflamed, but after a period of rest, the tissue can repair itself and return to normal. The major problem arises when there is such a short rest period (if any) that the body cannot repair the microscopic tears that appear in tendons and ligaments.
  • Sports: High-impact movements involving various sports can put you at risk for a harmful injury. Falling forward and extending your arms to break your fall, which can lead to a sprained or strained wrist.
  • Osteoporosis: osteoclasts and osteoblasts break down and build bone, respectively. When the body produces too little bone or breaks down too much bone, the joint structure can be weakened and the risk of pain and fracture increases. Interestingly, wrist fractures are often the first sign of osteoporosis.
  • Pregnancy: the prevalence of carpal tunnel syndrome in pregnancy has been as high as 62%.
  • Rheumatoid arthritis: I can cause inflammation of the wrist joint.

Diagnosis of hand and wrist pain

Dr. Miguel Alfonso Porcar a orthopedic specialist in wrist and hand pathologies and injuries, performs the diagnosis.

To diagnose the cause of wrist pain, the doctor will conduct a comprehensive physical examination, including wrist manipulation to check for numbness, pain, or tingling sensations; evaluate grip strength; and assess range of motion.

In addition, the doctor may perform or order imaging tests, which may include:

  • X-rays to evaluate the existence of bone and joint fractures.
  • CT scans to obtain more detailed images of bone fractures not visible on radiographs
  • Electromyography to assess nerve entrapment.
  • MRI to identify small fractures and soft tissue injuries.

Treatments for hand and/or wrist pain

There are a variety of treatments available for people experiencing wrist or hand pain and injury.

Initially for most pathologies we can start the use of analgesics and anti-inflammatory drugs, as well as the application of an ice pack on the affected area to relieve pain and swelling caused by a wrist injury.

In many cases we recommend strengthening and stretching exercises to help improve range of motion and strengthen the wrist muscles.

In the treatment of more complex cases with chronic pain in the wrist and/or hand, we can perform infiltrations with corticosteroids or hyaluronic acid, platelet-rich plasma (PRP) injections , autologous serum rich in cytokines and other forms of regenerative medicine, as options prior to surgery.

Arthroscopic or open surgery

In cases in which after the application of the previous treatments we do not obtain the desired results, or there is severe pain or inability to move, Dr. Alfonso Porcar may recommend arthroscopic surgery or open surgery depending on the patient’s characteristics.

Hyaluronic Acid Infiltrations

Hyaluronic acid infiltrations are indicated for patients with osteoarthritis or degenerative changes of the cartilage, potentially delaying disease progression and even the need for joint surgery.

It is commonly used in the treatment of osteoarthritis of the wrist, improving your quality of life and slowing down the progression.

    Autologous Cytokine-Rich Serum

The Autologous Serum Rich in Cytokines (Q-rem cytokine) is effective for treating hand and wrist conditions in the early stages of degeneration, reducing pain due to its natural anti-inflammatory properties.

We inject an adequate amount of platelet concentrates associated with anti-inflammatory cytokines into the damaged joint. These cytokines are responsible for reducing inflammation and initiating the regenerative cycle.

Autologous serum rich in cytokines achieves up to 75% pain reduction for more than one year in patients with osteoarthritis.

    Platelet-Rich Plasma (PRP) Treatment

PRP infiltrations stimulate the natural healing response, promoting recovery of mobility, strength, and normality in injured tissues.

This biological therapy induces healing mechanisms rather than replacing the injured area with a prosthesis or artificial joint.

    Key points about PRP treatment:

    1. PRP is a blood-derived product created from the patient’s own blood.
    2. It accelerates recovery and healing of tendon, joint, and muscle injuries.
    3. PRP is effective in treating sports injuries in the hand and wrist, and can be used as an adjuvant in surgery to improve outcomes.
    4. It reduces pain associated with osteoarthritis in the hands and wrists
