What is hip pain?
Hip pain is non-specific, i.e., it is not always felt precisely over the hip joint.
The pain may be referred in the hip joint and around (lateral region, lumbar area, groin, …), and the cause of the pain is multifactorial.
The exact area of your hip pain suggests the likely cause or underlying condition causing the pain.
- Pain in the groin area is more likely to be due to intra-articular pathologies.
- Pain in the external part of the hip, in the upper thigh or in the buttocks may be the consequence of muscular and/or tendon pathologies of the structures that make up the joint (extra-articular).
- Some pathologies that affect other parts of the body, such as the lower back or knees, also cause hip pain in a secondary manner.
Causes of Hip Pain Treatable by a Hip Specialist
The main cause of sudden hip pain is an injury caused by fracture of the femoral neck of the hip.
Hip fractures are common in the elderly due to their medical history, including osteopenia and/or osteoporosis.
Other causes of hip pain may be due to arthritis, bursitis, infection,…
- Gluteal tendinopathy: overuse injury of the hip tendons that are activated when we walk, stand and transfer weight from one leg to the other during walking.
- Hip bursitis treatment: The inflammation of one of the bursae of the hip (structure between the bony region and the insertion of the tendons of the musculature).
- Trochanteric bursitis: when the bursa that cushions the “greater trochanter” bone of the hip becomes inflamed. It usually hurts in the lateral region of the hip and it is treated with infiltrations for trochanteritis.
- Osteoarthritis on hip: degenerative joint disease occurs more frequently in the elderly. In osteoarthritis, the cartilage wears away, resulting in pain, swelling, stiffness and restricted movement in the affected joint.
- Bone marrow edema of the hip: inflammation or injury in the medullary area (inside the bone) of the hip, due to contusion. Predisposes to the appearance of microfractures and inflammation.
What are the symptoms of hip pain?
Hip pathologies can cause pain, and are a common concern for many people, especially as we age and maintain an active lifestyle.
Hip pain is usually associated with age. However, without proper care, anyone can develop wear and tear of the joint, and generate painful symptoms.
The following are common early symptoms of hip pathology:
- Hip pain or groin pain: This pain is usually located between the hip and knee. The pain may increase during exercise, sleeping or walking. It may lead to loss of hip motion.
Discomfort and pain during or after exercise is often the first sign of hip arthritis.
- Stiffness: A common symptom of hip stiffness is difficulty putting on shoes or socks. It may also be difficult to bend over or pick something up off the floor, even to rotate the hip in regular physical activity.
- Lameness: it is usually generated in an advanced stage of the pathology. The organism generates a different way of walking to help relieve more intense pain.
- Inflammation and sensitivity. It may occur internally or on the outside of the skin and muscle. If it becomes unbearable or does not disappear in a few days, contact your doctor immediately to rule out urgent pathologies.
These symptoms may vary depending on the cause of the pain.

Multidisciplinary pain management for complex cases may include:
- Ultrasound-guided injection of intra-articular corticosteroids or steroids and/or hyaluronic acid.
- Ultrasound-guided injection of prolotherapy for the treatment of ligament injuries.
- Intraosseous injections of platelet-rich plasma (PRP).
- Other forms of regenerative medicine include ultrasound-guided injections administered precisely at the site of injury.
- Arthroscopic surgery and/or total hip replacement are usually considered treatments of last resort.
Hyaluronic Acid Injections and/or Corticosteroid Injections
Injection of intra-articular high molecular weight hyaluronic acid stimulates the production of better quality synovial fluid, which helps protect the damaged joint.
It generates an artificial cushion that allows the patient’s cartilage damage to be reduced.
Hyaluronic acid injections may be the long-term solution you need. They are applied directly into the joint lubricating the structures, acting as a shock absorber in the cartilage and reducing inflammation.
It is commonly used in elderly and middle-aged patients, improving their quality of life and slowing down their evolution. And even avoiding the need for hip prosthesis.
Injection Treatment of the Hip with Platelet-Rich Plasma (PRP)
Ultrasound-guided injections of the hip with platelet-rich plasma (PRP) are a very effective procedure prior to surgery.
Use a concentrated dose of platelets prepared from your own blood.
Prior to the injection, the patient undergoes a blood extraction. After a period of processing, the platelet-rich plasma is obtained, which has been shown to have reparative and regenerative capacity.
Prior to the hip injection with PRP, we can administer a local anaesthetic injection to minimise discomfort for the patient during the procedure.
This is a safe technique, as the PRP substance is derived from the patient’s own blood, significantly reducing the risk of adverse effects following the injection.

QREM Cytokine (Cytokine Rich Serum) Hip Injection Treatment
QREM cytokine injections are a novel treatment for knee coxarthrosis.
We inject inside the damaged hip an adequate amount of platelet concentrates associated with anti-inflammatory cytokines, responsible for reducing inflammation and initiating the regenerative cycle.
Autologous serum rich in growth factors and cytokines achieves improved joint viscosity and up to 75% pain reduction for more than one year in patients with osteoarthritis.
Arthroscopic hip surgery
It is usually performed as a last resort, if necessary, when none of the previous treatments have worked and it is indicated by the patient’s pathology.
The camera projects images of the inside of the joint onto a monitor, allowing the physician to explore, evaluate and repair the injury.
It is indicated for the treatment of hip bursitis and femoroacetabular impingement (FAI).
Total hip replacement
Hip arthroplasty is a surgical procedure in which a hip joint damaged for various reasons is replaced with an artificial joint.
Hip joint replacement or total hip replacement (total hip arthroplasty) is used if conservative therapies do not give good results and the pain is persistent and very severe.
There are three types of hip prostheses:
- Cementless prosthesis: the artificial joint is attached or screwed to the bone. It then grows naturally into the bone (in young patients).
- Cemented prosthesis: the artificial joint is fixed to the bone by means of bone cement (in elderly patients).
- Hybrid prosthesis: the articular cavity is fixed in a natural way and the prosthesis stem is cemented.
The surgery lasts about an hour and is usually performed under epidural anesthesia. The patient can often leave the hospital the next day or two after surgery.
A total hip prosthesis lasts approximately 15 to 20 years on average before it needs to be replaced.
