What is carpal tunnel syndrome?

Carpal tunnel syndrome is a common condition that causes pain, numbness, tingling and weakness in the hand and wrist.

It occurs when there is increased pressure inside the wrist on a nerve called the median nerve.

This nerve provides sensation to the thumb, index and middle fingers, and half of the ring finger. The small finger(the “little finger”) should not be affected.

What is carpal tunnel?

Carpal tunnel is a narrow channel or tube in the wrist.

Similar to a tunnel through which one might travel in a car, this part of the wrist allows the median nerve and tendons to connect the hand and forearm.

The parts of this tunnel are:

  • Carpal bones: constitute the lower part and sides of the tunnel.
  • The ligament: at the top of the tunnel, the ligament is a strong tissue that holds the tunnel together.
  • Inside the tunnel are the median nerve and tendons.
Symptoms of wrist pain and/or hand pain

What are the causes of carpal tunnel syndrome?

Carpal tunnel syndrome can be due to any cause that enlarges the nerves or tendons, or makes the tunnel through which they run smaller.

  • An injury to the wrist or arm, such as a sprain or fracture.
  • Activities involving repetitive use (overloading) of the wrist and hand, including the use of vibrating tools.
  • Rheumatoid arthritis and other joint or connective tissue disorders.
  • Tendinitis.
  • Fluid retention during pregnancy or menopause.
  • Carpal tunnel cyst or tumor.
  • Other diseases such as diabetes, hypothyroidism, kidney disease with dialysis, abnormal growth of the hands (acromegaly).
  • Sometimes carpal tunnel syndrome occurs without any obvious cause or is idiopathic.

 

What are the symptoms of carpal tunnel syndrome?

The most common symptoms of carpal tunnel syndrome are:

  • Tingling or numbness in the fingers or palm that feels like pins and needles, mainly in the thumb and the next two fingers except the little finger.
  • Nerve pain in the wrist or hand, which may extend down the arm or fingers.
  • Weakness in the hands, making it difficult to grasp objects.
  • Swollen fingers.

Symptoms are usually worse in the hand that is used the most (the dominant hand), but the disease can affect both hands.

Over time, people with carpal tunnel syndrome may find that they slowly lose strength and movement in their hand and wrist.

That’s why it’s important to see your doctor if you think you may have carpal tunnel syndrome.

Risk Factors

People who are particularly prone and at higher risk of developing carpal tunnel syndrome belong to one of the following groups:

  • Women between 40 and 60 years of age.
  • Pregnant women.
  • People with arthritis.
  • People who put on weight quickly.
  • People who repeatedly use their hands in their work…

Diagnosis of the carpal tunnel syndrome

Dr. Miguel Alfonso Porcar performs the diagnosis of carpal tunnel syndrome, numb hands and/or trigged fingers.

Diagnosis is made through clinical history and physical evaluation.

We will assess your symptoms and then conduct additional tests to evaluate the speed at which the median nerve transmits signals through the carpal tunnel.

  • Tinel’s sign: In this test, the doctor taps the median nerve at the wrist to see if it produces a tingling sensation in the fingers.
  • Wrist flexion test (or Phalen’s test): in this test, the patient rests the elbows on a table and lets the wrist fall freely forward.

Individuals with carpal tunnel syndrome will experience numbness and tingling in their fingers within 60 seconds. The faster the symptoms appear, the more severe the carpal tunnel syndrome.

  • X-rays: we may request X-rays of the wrist if there is limited movement of the wrist, or evidence of arthritis or trauma.
  • Electromyography (EMG) and nerve conduction studies: These studies determine the function of the median nerve itself and its ability to control muscle movement.

Treatment of Carpal Tunnel Syndrome

Treatment of carpal tunnel syndrome depends on its cause.

Carpal tunnel syndrome can be treated with conservative approaches or surgery. Conservative treatments include:

  • Treat a related medical condition that may be causing the symptoms.
  • Use a wrist brace or splint to keep the wrist straight. Immobilization of the hand and wrist with a splint or wrist brace for 4-6 weeks especially at night.
  • Rest from symptom-provoking activities, application of ice packs to prevent swelling and anti-inflammatory medications to relieve pain.
  • Medications such as non-steroidal anti-inflammatory drugs.
  • Strengthening and stretching exercises once symptoms subside.

Corticosteroid injections

When the most conservative therapies do not alleviate the symptoms, Dr. Alfonso Porcar may recommend corticosteroid infiltrations using ultrasound-guided technique.

Corticosteroid injections are placed into the carpal tunnel to temporarily relieve the pain associated with carpal tunnel syndrome. The injections are intended to relieve pressure on the median nerve, which causes pain from the inflamed tendon.

Carpal tunnel surgery

Carpal tunnel syndrome can be treated with carpal tunnel release surgery.

The surgery involves a 3 cm incision in the palm and wrist area.

During surgery, the transverse carpal ligament will be dissected to relieve pressure on the median nerve and widen the carpal tunnel.

Postoperative care

Dr. Miguel Alfonso Porcar may suggest that you perform certain postoperative procedures for a better recovery and to avoid further complications.

  • Keep the hand elevated above the level of the heart to reduce swelling.
  • Use a splint.
  • Apply ice packs to the surgical site to reduce swelling.
  • Keep the surgical incision clean and dry.
  • Follow a rehabilitation plan with physical therapy exercises to regain wrist strength.

Causes of wrist pain or pain in the hand

Hand or wrist pain causes

Some of the most common causes of hand and wrist pain that a hand specialist can treat are sprains and strains.

A sprain refers to an injury to a ligament, while a strain refers to a muscle injury. Sprains and strains occur due to excessive force applied during a stretching, twisting or pushing action.

Other causes of pain in the hands and/or wrists include:

  • Osteoarthritis on hands: A degenerative joint disease that is the most common type of arthritis affecting older people. It causes the cartilage, the tissue that cushions and protects the ends of the bones in a joint, to wear away, resulting in painful bone-on-bone contact.
  • Arthritis in thumb: Arthritis affecting the base of the thumb, i.e. the progressive destruction of the articular cartilage between the trapezium and the first metacarpal.

  • Trigger fingers: A condition in which the finger locks or catches if you try to straighten or bend it.
  • Carpal tunnel surgery: Carola tunnel syndrome is characterized by numbness or pain in the thumb and first three fingers. It occurs when the median nerve is compressed at the wrist. It is usually a common complaint in people who use their hands for prolonged periodsin particular occupations, such as computer work.
