What is thumb tendonitis or De Quervain’s tenosynovitis
One of the most common types of wrist tendon inflammation, De Quervain’s tenosynovitis (also called De Quervain’s tendinosis), is a painful condition in which the tendons of the thumb become inflamed and impede the mobility of the thumb due to pain.
When this occurs, the ability to move the thumb and wrist normally may be lost.
Such pathology can be caused by overuse, repetitive gripping or associated precipitating inflammatory medical conditions such as arthritis.
As a general rule, tendons slide easily through a tunnel of tissue called a sheath.
The sheath holds the tendons in place next to the bones of the thumb in order to perform the function of moving the fingers with which they are associated.
Inflammation of the tendons and/or thickening of the sheaths causes friction. The tendons can no longer glide easily through their sheaths. When this occurs, certain movements of the thumb and wrist become more difficult to perform.
Causes of De Quervain’s tenosynovitis
Quervain’s tendonitis is one of the most common causes of inflammation of the tendon lining (sheath).
This condition most frequently affects women, in an age range between 40 and 60 years old.
This pathology can be caused by several factors, among them:
- Excessive use of the thumb. Typical in manual workers.
- A direct blow to the thumb or secondary to a fracture of the distal radius.
- Inflammatory conditions (including arthritis).
- Infectious cause (local or systemic).
- Without a specific cause(idiopathic). The most frequent
Activities that require lateral movement of the wrist while grasping a heavy object with the thumb may aggravate this condition.
Hobbies and sports using this movement include:
- Gardening.
- Racquet sports.
- Skiing.
- Carpentry.
Symptoms of De Quervain’s tenosynovitis
The symptoms described may be the following:
- Pain and tenderness along the inner region of the wrist, over the thumb.
- Pain that worsens when using the hand and thumb.
- Pain that comes on suddenly or develops over time.
- Pain that moves to the thumb or from the wrist to the lower arm (forearm).
- Snapping or popping sensation in the wrist when moving the thumb.
Diagnosis of Dequervain’s tendinitis

De Quervain’s Tenosynovitis Treatments
There are several treatment options. In almost all cases we will start with the most conservative (non-surgical) treatments first, followed by ultrasound-guided infiltrations.
Finally, surgical treatment is also an option.
Conservative treatment of De Quervain’s tenosynovitis
Initially we will start with RICE treatments, based on rest, ice application, compression by bandaging and elevation.
- Antalgic orthoses or splints: to rest the wrist and thumb, preventing their mobilization. Splints are usually worn 24 hours a day for four to six weeks.
Many people find it helpful, especially at night when a sore thumb can wake them up or to avoid a numb thumb in the morning.
- Apply ice to the thumb and wrist area to reduce swelling.
- To eliminate pain and inflammation, it is possible to modify or limit your activities, avoiding any type of movements that involve repetitive use of the thumb.
- We may also recommend non-steroidal anti-inflammatory drugs (NSAIDs) to relieve pain and prevent inflammation.
Corticosteroid injections
Steroid injections into the tendon sheath may be considered. Ultrasound-guided injections to localize the exact point of injury.
This way we avoid skin discoloration or cutaneous fat atrophy, especially in women, caused by too superficial corticoid injection.
Platelet Rich Plasma Treatment
Multidisciplinary pain management for complex cases with chronic tendonitis may include platelet-rich plasma (PRP) injections and other forms of regenerative medicine as options prior to surgery.
- Use a concentrated dose of platelets prepared from your own blood.
- After a period of blood processing, Platelet Rich Plasma is obtained, which has proven to have reparative and regenerative capacity.

Surgery for De Quervain’s Tenosynovitis
If both conservative treatment and PRP infiltrations do not resolve the condition and symptoms persist for 6 to 12 months, Dr. Miguel Alfonso Porcar may recommend surgery.
It is an outpatient procedure usually performed under local anesthesia or mild sedation.
The goal of this surgery is to eliminate pain and restore range of motion of the thumb and wrist.
During surgery, Dr. Miguel Alfonso Porcar makes a small incision in the sheath through which the tendons pass. This allows more room for the tendons to slide easily through the sheath.
After recovery, we will perform an exercise program to strengthen the thumb and wrist. Recovery time varies depending on age, general health and duration of symptoms.
In cases that have developed gradually, the pathology is often more difficult to manage. Symptoms may take longer to disappear and the thumb and wrist may take longer to regain their range of motion.
After treatment, it can be prevented by changing the activities that cause pain.
