Why doesn’t knee osteoarthritis pain go away on its own?
Knee osteoarthritis causes chronic pain because an inflammatory cycle is maintained inside the joint that no conventional treatment has been able to interrupt in a lasting way.
The synovial membrane becomes inflamed (synovitis). This inflammation releases enzymes that degrade the cartilage. The remains of the damaged cartilage fall into the synovial fluid and reactivate the inflammation. And the cycle starts all over again.
As long as that cycle is active, the pain returns. No matter how many pills you take or how many corticosteroid or hyaluronic acid injections you have had.
The polyacrylamide hydrogel acts exactly where other treatments fail: it integrates into the synovial membrane and thickens it, creating a physical barrier that interrupts the inflammatory cycle from within.
As the synovial membrane thickens with the integrated hydrogel (the liquid prosthesis), the inflammatory cells are physically separated and cannot maintain the cycle.
The result is a joint with better cushioning, less stiffness and without the chronic inflammatory stimulus that maintained the pain.
When is intra-articular hydrogel liquid prosthesis indicated?
This treatment is indicated for adults with osteoarthritis of the knee, hip, ankle… of moderate or severe degree.
It is especially suitable when:
- The pain persists despite physiotherapy, anti-inflammatory drugs or previous hyaluronic acid or PRP infiltrations.
- Functional limitation affects daily life: walking, climbing stairs, getting up from a chair, sleeping.

In which cases is liquid prosthesis not recommended?
This treatment is not suitable if there is active infection in the joint, if the patient has an implanted knee prosthesis, if he has undergone arthroscopy in the last 6 months or if he has uncontrolled anticoagulant treatment.

What is the liquid knee prosthesis?

What are the effects of intra-articular hydrogel on osteoarthritic knee?

What does intra-articular hydrogel treatment consist of?
The treatment is an outpatient procedure, minimally invasive, performed in consultation under ultrasound guidance. It is not a surgery. It does not require general anesthesia or hospitalization.
Echoguided infiltration process
Previous preparation: 1-2 hours before, oral antibiotic prophylaxis is administered to reduce the risk of infection. Local anesthesia is applied at the puncture site.
Ultrasound-guided infiltration: Under ultrasound control and strict aseptic conditions, the specialist injects 6 ml of the hydrogel directly into the joint space through a single needle. Ultrasonography guarantees exact placement.
Recovery: The patient can go home the same day and resume his activity in 24-48 hours. The first results can be seen after the fourth week.
Results of the liquid knee prosthesis
After infiltration, the hydrogel is distributed in the joint fluid and begins to adhere to the synovial lining. In approximately one month it is completely integrated into the synovial membrane. The results continue to consolidate until 3-6 months.
After the infiltration, relative rest is recommended for 48 hours and paracetamol can be taken in case of discomfort. Anti-inflammatory drugs should not be taken during the first days so as not to interfere with the integration process.
Clinical outcomes: long-term evidence
This hydrogel has the most comprehensive clinical research program of any intra-articular treatment available today, with follow-up of the same patients for up to 4 years after a single injection.
- At 4 weeks: significant reduction in pain, exceeding the minimum clinically important difference of 9 points on the WOMAC scale.
- At 6 months: 2 out of 3 patients have a positive clinical response (OMERACT-OARSI criteria).
- At one year: 74% of patients improve. In patients under 70 years of age, 80%. In patients under 60 years of age, 94%.
- At 2 years: reduction of pain maintained with statistical significance.
- At 3 years: clinical and statistically significant efficacy confirmed.
- At 4 years: it is the only intra-articular implant with efficacy data at this time horizon in the entire history of knee osteoarthritis treatments.
Compared directly to hyaluronic acid in a randomized, double-blind trial, the hydrogel showed equal results at 6 months and significantly superior results at 1 year in patients over 70 years of age: the most frequent patient profile with severe knee osteoarthritis.
Treatment safety
This treatment does not present significant risks for the patient. It is performed with a CE marked certified medical device. The same polyacrylamide material has been injected in more than one million medical procedures worldwide for various indications over more than two decades.
Blood and joint tissue do not come into contact with air or contaminated components at any time. The procedure complies with all safety conditions.
Possible side effects
- Pain or sensation of pressure in the knee during the first 24-72 hours.
- Transient joint inflammation.
In all clinical studies performed, no intra-articular infection or allergic reaction has been reported.
If arthroplasty surgery is subsequently necessary, the hydrogel does not hinder or condition the intervention: no related surgical complications have been reported in patients operated on after treatment.
Differences with other treatments for osteoarthritis of the knee
Intra-articular hydrogel vs. hyaluronic acid
Hyaluronic acid lubricates the joint temporarily but is absorbed by the body in weeks or months. It is necessary to repeat the treatment periodically.
The polyacrylamide hydrogel is not absorbed and remains integrated in the synovial tissue, with documented effects up to 4 years. In addition, it acts on synovial inflammation, not only on lubrication.
Intra-articular hydrogel vs. PRP
PRP stimulates the body’s own repair mechanisms and is useful in many contexts of mild to moderate osteoarthritis. However, it does not act on the synovial membrane permanently and its effect has a more limited duration.
The hydrogel is especially indicated when PRP has not given sufficient results or when a more lasting effect is sought in moderate or severe osteoarthritis.
Intra-articular hydrogel vs. knee prosthesis.
Knee replacement surgery is the treatment of last resort in very advanced osteoarthritis.
